【摘要】 目的 观察血液净化联合中药治疗重型肝炎合并肾功能衰竭对降低其病死率的作用。方法 对27例重型肝炎合并肾功能衰竭患者采用血液净化疗法联合中药治疗。结果 27例中8例存活，19例死亡，4例直接死于肾功能衰竭。结论 该疗法在降低重型肝炎合并肾功能衰竭病死率可能有一定效果，但应早期诊断、早期治疗，并加强原发病—肝功能衰竭的治疗。

    【Abstract】 Objective To evaluate a combination treatment withhemopurification and Chinese herbs in reducing mortality of severe viral hepatitiscomplicated acute renal failure.Methods Subjects in the present studyconsisted of 27 patients (21 male, 6 female, mean age 41.5 years) with severe viralhepatitis complicated acute renal failure, 12 with simultaneously presented cerebraledema. The Patients received a combined treatment with hemopurification and Chinese herbsfrom 1990 to 1996. Three types (PE, CAVHD, HD) of hemopurification were selected, andChinese herbs were used by oral administration or enema before and after hemopurification.Results After treatment, 8 of 27 patients survived, 7 of 15 patients with severe viral hepatitisand acute renal failure survived. Conclusions Hemopurification, PE+CAVHD OR HD can clearpoisonous substances in serum, decrease the serum BUN, CR, improve renal function, reducecerebral edema, and adjust electrolyte and osmotic pressure. Chinese herbs can helpimprove micro-circulation before and after hemopurification. The results suggest it isnecessary to diagnose and treat severe viral hepatitis early, and the combinationtreatment is worth attempting in reducing mortality of severe viral hepatitis complicationacute renal failure.
