五种肌腱缝合方法的生物力学研究

摘要 】目的通过比较5种肌腱缝合方法的生物力学特性,以指导临床工作。方法将新鲜成年猪后足50只,随机分成5组,解剖暴露第二趾深屈肌肌腱后,并在Ⅱ区横断,第1组行改良Kessler缝合,第2组行津下缝合,第3组行双改良Kessler缝合,第4组行津下加双改良Kessler缝合,第5组行汤氏缝合。用Instron力学测定仪测定每组缝合方法的2mm间隙形成的负荷、最大负荷及功耗,并行统计学分析(ANOVA)。结果汤氏缝合法的2mm间隙形成的负荷、最大负荷及功耗最大,改良Kessler及津下缝合法最小。汤氏缝合法与其他4组相比较统计学差异有显著性意义(P<0.01)结论汤氏缝合法修复的肌腱抗张强度最大.

    Biomechanical Comparisonof Five Flexor Tendon Repair Techniques

Abstract 】 Objective This study wasdesigned to compare the special biomechanical features of five different suture techniqueswhich were used clinically for tendon repair. Methods 50 flexor profundus tendons of thetoes of the hindfeet of the pigs were randomly allocated into five groups. The tendons ineach group were transected in zoneⅡ,and were repaired by the following five suturetechniques:(1) Modified Kessler;(2) Tsuge;(3) Double Kessler;(4) Modified Kessler+ Tsuge;(5)Tang’s suture. Instron testing machine was used to test 2 mm-gap formationforce and failure load. Maximum work was also calculated for comparison between eachgroup. Results Tang’s suture technique has the greatest tensile strength with its 2mm-gap force and failure load almost one fold larger than group 1 and 2, 0.5 time larger thangroup 3 and 4. The maximum work was also significanty greater than that of other foursuture techniques(P<0.01 or P<0.001).Conclusion Tang’s suture technique wasrecommended for the clinical repair of tendon in zone Ⅱbecause of its reliable tensilestrength.
