碱性成纤维细胞生长因子促进急性坏死性胰腺炎肠粘膜损伤修复的实验研究

【摘 要】 目的 观察基因重组碱性成纤维细胞生长因子(bFGF)对急性坏死性胰腺炎(ANP)犬肠粘膜损伤修复的影响。方法 经主胰管注入牛磺胆酸钠和胰蛋白酶复制犬ANP模型,观察ANP时及bFGF治疗后肠粘膜组织结构、蛋白、DNA和丙二醛含量改变,检测血浆内毒素和内皮素-1(ET-1)水平,脏器作细菌培养。结果 对照组胰腺腺泡及肠粘膜无异常。ANP组胰腺腺泡大片出血、坏死,大量中性细胞浸润,肠粘膜绒毛脱落、变短等。bFGF治疗组胰腺腺泡仅残留极少数坏死,大部分腺泡结构恢复正常;肠粘膜组织损害明显减轻,肠粘膜绒毛宽度、高度和面积均显著增加,蛋白、DNA含量增加,丙二醛显著下降,血浆内毒素水平下降50%~67%,ET-1水平亦显著降低,脏器细菌移位率减少50%。结论 bFGF能显著促进ANP犬肠粘膜损伤的修复,保护肠屏障功能,减少肠道内毒素和细菌移位。

EFFECTS OF BASICFIBROBLAST GROWTH FACTOR ON REPAIRING INJURY OF INTESTINAL MUCOSA IN ACUTE NECROTICPANCREATITIS

Abstract Objective To observe the effects of basic fibroblast growth factor (bFGF) on repairing injury of intestinal mucosa in acute necrotic pancreatitis (ANP).Methods Sixteen dogs of ANP animal modelwere made by injection of 5% sodium taurocholate (0.5 ml/kg) with 3 000 U/kg trypsin intothe pancreatic duct. The mucosa structure, content of protein, DNA andmalondiethylaldehyde (MDA) were observed after ANP and treatment with bFGF, and the plasmalipopolysaccharide and endothelin-1 were detected. The organs of dogs were made tobacterial culture. Ileal mucosa was collected for histological and ultrastructuralstudies.Results The results showed that after treatment with bFGF, the injury ofintestinal mucosa in ANP was abated. The length, height and area of mucosa microvillus,the content of DNA and protein of ileal mucosa were significantly increased, while theplasma endothelin-1 and lipopolysaccharide were reduced. The organ bacterial translocationrate was also decreased in 50%. Conclusion bFGF has good effects on abating injury of intestinal mucosa, protecting gut barrier function, reducing the incidence of lipopolysaccharide andbacterial translocation after ANP.
