摘 要 目的:探讨声学密度定量(AD)技术在评价缺血、梗塞后心肌内在收缩性能方面的临床应用价值。方法:对70例冠心病患者(分为缺血组与梗塞组)及30例正常人相应部位心肌及左室腔血液的CVIB值进行测定。结果:与正常节段比较,缺血及梗塞节段的CVIB值明显减少, N-Delay明显延长;14例患者梗塞节段的CVIB曲线与正常节段呈镜向改变;三组左室腔血液及二尖瓣环(MVR)的CVIB值无显著差异;结论:AD技术能客观反映缺血、梗塞后心肌内在收缩性能的变化,具有较高的敏感性和特异性,对于临床鉴别心肌收缩异常具有一定的诊断意义。
A Study of MyocardialContractility in Ischemia or Infarction by Acoustic Densitometry

ABSTRACT Objective:To evaluate thediagnostic value of acoustic densitometry (AD)technique in detecting myocardialcontractility secondary to ischemia or infarction. Methods:Seventy patients withcoronary artery disease(CAD)and 30 normals were involved in this study.Measurements ofcyclic variation of integrated backscatter (CVIB)were obtained in corresponding regionalmyocardium and blood of the left ventricle.Results:In comparison with the normal regionalmyocardium,the values of Normalized-Delay increased but CVIB decreased significantly incorresponding ischemic or infarcted myocardium in CAD groups(P<0.005).There were nosignificant difference of the CVIB values in MVR or blood in the threegroups.Conclusion:Acoustic densitometry can detect the changes of myocardialcontractility.It is useful for identification of abnormal myocardial contractility.
