【摘要】 目的 探讨重症监护病房(ICU)中长期使用机械通气(≥14天，LTMV)患者治疗中应关注的一些问题。 方法 回顾我院1998年10月～2004年12月ICU收住的34例长期机械通气患者在呼吸机相关肺炎(VAP)、抗生素使用情况、营养支持、机械通气相关并发症、撤机等方面问题进行分析。 结果 34例患者中出现呼吸机相关肺炎28例(82.4%)，营养不良15例(44.1%)，MODS12例(35.3%)，撤机困难8例(23.5%)，感染性休克8例(23.5%)，气管出血2例，气管导管阻塞2例，气管狭窄1例，气胸1例，肺间质纤维化2例，肾衰3例，死亡12例(35.3%)。 结论 LTMV患者住院时间延长，医疗费用增高，生命质量降低，死亡率较高。LTMV治疗过程中应注意加强防治VAP，加强营养支持，制定合理通气策略，尽快撤离呼吸机。
