【摘要】 目的 探讨去大骨瓣减压术治疗重型颅脑损伤之临床效果。 方法 将以大骨瓣减压术治疗的30例为治疗组，对照以传统骨瓣减压术治疗的30例，比较两组疗效。两组术前术后均采用脱水、适度/过度换气、抗炎、止血、护脑等治疗。半年后统计疗效。 结果 治疗组良好56.66%，中残16.66%，重残6.66%，植物生存3.33%，总有效率83.33%，死亡率16.66%;对照组良好30%，中残13.33%，重残10%，植物生存6.66%，总有效率53.33%，死亡率40%。两组总有效率比较，差异有显著性(χ 2 =6.23，P<0.05)。 结论 大骨瓣减压术治疗重型颅脑损伤能提高成功率，降低死亡、致残率，提高生活质量。
