【摘要】 目的 将慢性病自我护理模式应用于糖尿病患者康复过程，探讨其对提高患者自我护理能力的实践意义和价值。 方法 利用评估-计划实施-再评估的方式对53例住院糖尿病患者进行有针对性的护理干预，比较两次评估患者自我护理能力的区别。 结果 两次评估患者的自我护理能力有很大的不同。 结论 自我护理模式应用于糖尿病护理有着积极的意义，能提高糖尿病患者的自我护理能力。

    【Abstract】 Objective The purpose of this study was to describe the mode of chronic self-care in clinical nursing on diabetes;and to explore it's application and value of the more ability of the patients'self-care.Methods The mode of chronic self-care(evaluate-plan-implement-reevaluate)was used to nurse53inpatients with dia-betes;and then we compare the differences of two evaluation.Results It was obvious that the ability of the the pa-tients with patients's self-care in two evaluations were different.Conclusion The mode of chronic self-care in clinical nursing on the patients with diabetes was suited;and it could advance the ability of the patients'self-care. ......
