【摘要】 目的 探讨老年患者瓣膜替换术围术期处理方法。 方法 总结43例老年心脏瓣膜置换病例。39例置换机械瓣，置换生物瓣4例。行二尖瓣置换术22例，主动脉瓣置换术8例，二尖瓣、主动脉瓣双瓣置换术12例，改良Bentall手术1例。早年的1例术中发现冠状动脉前降支狭窄，近年的21例术前常规行冠状动脉造影术3例证实存在冠状动脉狭窄者同期行冠状动脉搭桥术。 结果 全组死亡3例，其中术后早期死于重度低心排1例，左室后壁破裂1例，多器官衰竭1例。随访6个月～8年，共随访32例，随访率80%，心功能均有明显改善，无远期死亡。 结论 单一的老龄因素已不应视为老年瓣膜病行瓣膜置换术的禁忌证;老年性瓣膜病患者术前应常规行冠状动脉造影术，有合并冠状动脉狭窄者，要同期行冠状动脉搭桥手术;老年患者各脏器功能储备减退，术后及时处理低心排、控制血糖，维护呼吸、肝与肾功能。防治各种并发症是术后顺利恢复的关键。
