全身亚低温对家兔持续性急性心肌梗死24h梗死面积的研究

     摘 要:目的:探讨亚低温疗法对家兔持续性心肌梗死24h梗死面积的影响。方法:13只家兔随机分为常温组和亚低温组,开胸结扎心脏前降支建立急性心肌梗死模型。亚低温组通过物理降温使结扎后30min体温降到35℃以下,保持在33~35℃之间;常温组体温维持在38℃以上,取心染色切片,计算梗死心肌和左室心肌重量。结果:①常温组恶性心律失常发生率高于亚低温组(1/7比0/6),死亡率也高于亚低温组(1/7比0/6)。②亚低温组梗死24h血清CK- MB活力低于常温组(34119.33±11532.69比46950±12915U/L;P=0.099)。③亚低温组梗死心肌重量与常温组相近(0.57±0.05比0.60±0.09;P>0.05)。占左室心肌重量的比例也相近(16±1%比16±1%;P>0.05)。结论:亚低温疗法并不能缩小家兔持续性心肌梗死24h的面积,但可以减少心肌酶的释放,延缓心肌细胞的坏死速度。

    Experimental Study of Continuous Coronary Artery Occlusion for 24hours on the Acute Myocardial Infarction Size of Body Mild Hypothermia in Rabbits

    Abstract: Objective: To study the effect of body mild hypothermia on the myocardial infarction size in continuous coronary artery occuluded for 24 hours in rabbits. Method: 13 rabbits were randomly divided into 2 groups:mild hypothermia group and control group. Each group underwent 24 hours of left anterior descending coronary artery occulusion. Core temperatures were measured with thermistor. The mild hypothermia group received ice cooling around the body and the core temperature was dropped to 33~35℃ after occluded for 30 minutes, while the control group's body temperature were kept above 38℃.The infarct area were determined with triphenyl tetrazolium Chloride(TTC).Result:① The frequency of malignant arrtythmia of the control group was higher than that in the mild hypothermia group(1/7VS.0/6),and the mortality was also higher compared with the control group(1/7Vvs.0/6).②After 24 hours coronary occlusion, the changes of serum CK- MB activities in mild hypothermia group were lower in the control(34119.33±11532.69vs.46950±12915u/l;p=0.099).③The size of infarct myocardium of the mild hypothermia group were almost the same as that of the control group (0.57±0.05gvs.0.60±0.09g;P>0.05 and of the left ventricle size(16±1% vs.16±1%;p>0.05)were almost the same as that of the control group. Conclusion: Body mild hypothermia cannot reduce the size of infarction in continuous coronary artery occlusion for 24hours,but it can reduce the release of the myocardial enzyme and delay the necrosis speed of the myocardial cells. ......
