老年人巨幼细胞性贫血31例临床分析

摘要: 目的：探讨老年人巨幼细胞性贫血(MBA)的病因、发病规律及临床特征。方法：对31例老年MBA患者的发病原因、临床特征及疗效进行回顾性分析。结果：31例老年巨幼细胞性贫血患者占同期收住所有的巨幼细胞性贫血患者的36.1%，男性∶女性为3.43∶1，城镇居民25例(80.7%)；伴有慢性胃肠疾病者占48.4%；临床上重度贫血患者占32.3%，16%的患者感染结核菌，87.5%的患者乳酸脱氢酶(LDH)升高，46.7%的患者胆红素升高；29%的患者被误诊；31例患者用叶酸、维生素B12治疗疗效均显著。结论：老年MBA患者的发病率增加的原因可能是人口老龄化、慢性消化道疾病及老年人常控制饮食和/或过度烹饪加工食品等，老年人MBA患者贫血多较重且易误诊，及时诊断与治疗预后良好。关键词：贫血；巨幼细胞性；老年人
A clinical analysis of 31 cases senile megaloblastic anemia

Abstract: Objective: An analysis of the etiology factors and clinical feature of senile megaloblastic anemia. Methods: The 31 senile patients with megaloblastic anemia were analysed retrospectively. Results: The 31 cases with senile megaloblastic anemia accounted for 36.1% of total megaloblastic anemia patients in our hospital in the same period. Males were predominate with a male to female ratio 3.43∶1. 80.7% of senile megaloblastic anemia were the town inhabitant. 48.4% of them suffered from chronic gastric disorders. 32.3% senile patients showed severe anemia. 16% of senile megaloblastic anemia suffered from tuberculosis. Lactate dehydrogenase and bilirubin increased in 87.5% and 46.7% senile patients respectively. 29% of cases with senile megaloblastic anemia were misdiagnosed. The response to folic acid and vetamin B12 of all 31 cases was well. Conclusions: In recent years the incidence of megaloblastic anemia in elder is increaed. The main cause maybe is the increase of aging people, chronic gastricintestinal disorders, their diet control and/or their food cooking excessively. The patients with senile megaloblastic anemia often have severe anemia, and they are easy to be misdiagnosed. The prognosis of them would be well if the diagnosis and treatment have been taken promptly. ......
