内分泌治疗中晚期前列腺癌49例临床分析

摘要：目的： 探讨内分泌治疗中晚期前列腺癌患者的临床疗效。方法：对49例中晚期前列腺癌患者的临床资料进行回顾性分析。49例分为A、B、C 3组,A组21例行双侧睾丸切除+药物去势,B组23例给予持续性药物雄激素阻断,C组5例行间歇性药物雄激素阻断,分析3组的症状缓解率、影像学评价有效率、前列腺特异抗原(PSA)反应率及1、2、3年的生存率。 结果： A、B、C 3组的症状缓解率分别为52.38%、52.17%、80.00%，影像学评价有效率分别为71.43%、69.57%、100.00%，PSA反应率均为100%。A、B组1、2、3年生存率分别为90.48%、80.95%、80.95%,95.65%、86.96%、82.61%,C组近期疗效满意，1年生存率100%，间歇期生活质量提高，治疗费用降低。结论：内分泌治疗是中晚期前列腺癌的有效治疗手段之一；药物去势与手术去势疗效无明显差异；间歇性内分泌治疗近期疗效满意，可提高病人生活质量，降低治疗费用。关键词：前列腺癌;内分泌治疗;间歇性雄激素抑制;激素依赖
The clinical analysis on endocrine therapy of late prostate cancer

Abstract: Objective: To assess the effect of endocrine therapy in patients with late prostate cancer. Methods: Fortynine patients with prostatic cancer, 14 patients at pathologic stage C, 35 stage D, were analyzed retrospectively. Twentyone of them were treated by bilateral orchidectomy and castration of drug (group A); twentythree of them were treated by consist castration of drug (group B);five patients were treated by intermittent androgen suppression. Analyze the three groups by symptom release rate, radiological evaluate, PSA reactive rate, life time, and life quality, intermittent period of group C. Results: The response rates of 3 groups were 52.38%、52.17%、80.00% differently；the effective rates by radiology were 71.43%、69.57%、100.00%； PSA response rate was 100.00% of the same; the group C acquired content result, such as better life and less cost. Conclusions: Endocrine therapy is one of the effective treatment methods of late prostate cancer; there is no clearly differences between drug castration and surgical castration; intermittent endocrine therapy have satisfied short term effect, can improve life quality of patients, and can reduce the cost of treatment, thus, worth further observe. ......
