多层螺旋CT血管造影在颅内疾病中的应用价值

     【摘要】 目的 探讨多层螺旋CT脑血管造影在颅内病变中的应用价值。方法 采用GE Light SpeedTM 4.X 16层螺旋CT机对怀疑有颅内病变的45例患者行CTA检查,将扫描的原始图像导入Sun Workstation 4.0工作站利用后处理软件行MPR、MIP、VR重建。对血管显示情况进行评价。结果 45例中,正常的14例,动脉瘤14例,血管狭窄及Moyamoya病各1例,动静脉畸形(AVM)2例,脑膜瘤8例,听神经瘤1例,胶质细胞瘤4例。脑动脉主干及其1~3级分支显示率为100%,4级分支显示率为62.2%(28/45)。动脉瘤14例,清晰显示了瘤体、瘤颈和载瘤动脉;2例动静脉畸形,显示了AVM的畸形血管团及其供血动脉和引流静脉;Moyamoya病1例,清晰显示了基底节区的烟雾血管;8例脑膜瘤和1例听神经瘤显示了肿瘤与周围血管和颅骨间的关系。结论 多层螺旋CT血管造影的图像质量高,二维及三维重建图像能清晰显示脑血管更精细的结构,对脑血管病变的诊断及颅内病变与血管的三维空间关系的定位有重要价值。

    Study of the value of multislice spiral CT angiography for intracranial diseases

    【Abstract】 Objective To study the advantages of multislice spiral CT angiography(MSCTA),to evaluate MSCTA in diagnosis of central nervous system disease.Methods Forty five patients suspected of central nervous system disease were examined by GE Light SpeedTM 4.X 16-slice spiral CT.Source images were transferred into Sun ADW4.0 workstation,then processed with MIP,VR,MPR techniques.Results The cases included normal(n=14),cerebral aneurysm(n=14),vessel stenosis(n=1),moyamoya disease(n=1),arteriovenous malformation(n=2),meningioma(n=8),acoustic Schwannomas(n=1),gliomas(n=4).All main trunks and 1~3 branches of cerebral arteries in all cases were well visualized,and four branches were well displayed in 28 cases.Aneurysms body,neck,source vessel and their relationship were surely and clearly displayed in 14 cases of aneurysms.In two cases of AVM,the vascular malformation and its feeding artery,draining veins were clearly showed.The smog vessels in the basal ganglia were showed clearly in a case of moyamoya disease.In 8 cases of mengioma and a case of acoustic Schwannomas,CTA clearly showed the tumor,near vessels,skull structure and their relationship.Conclusion The images of 16-slice spiral CTA are perfect,and the reconstruction images can display more tiny structure of brain vessels,its value for the diagnosis of cerebral vascular diseases and displaying relationship between intracranial diseases and near vessels is high. ......
