转移盘牵引成骨整复山羊下颌骨节段性缺损的组织学评价

    摘 要:目的 研究双界面牵引成骨(BDO)整复山羊下颌骨节段性缺损的成骨方式、新骨结构及各骨段界面的结合. 方法 山羊2只,单侧下颌骨节段性缺损BDO整复,固定期2mo,3mo分别处死动物,整复侧下颌骨取材,制备组织切片,HE、三色法染色,观察. 结果 新生骨高度、宽度与正常下颌骨相同,骨质密度较正常下颌骨高,骨小梁沿牵引成骨方向规律排列.牵引形成的新生骨与远中骨残端界面为骨性结合.新生骨内含下牙槽动脉,其周围为平行排列的牵引形成新骨.新生骨与转移盘为骨性结合,转移盘处于改建过程,与近中骨残端之间为骨性结合.新生骨为直接膜内成骨、未发现软骨内成骨. 结论 组织学研究证实BDO整复下颌骨节段性缺损方法可行,治疗效果确切,具备临床应用条件.

    Histologic evaluation of mandibular defects recon┐struction by bifocal distraction osteogenesis in goats

    Abstract:AIM To observe the structure of distracted bone and interfaces between proximal mandible,to transport disk and reconstructed bone of mandibular defects in bifocal dis-traction osteogenesis(BDO)in goats.METHODS Two goats with semi-mandibular continual defects were recon-structed by BDO.They were sacrificed and the mandibles harvested2or3months after the distraction period.All specimens were decalcified and embedded in paraffin.Repre-sentative sections were stained with either hematoxylin-eosin or Trichrome-staine.RESULTS Macroscopic and histologic evaluation of the regrown segments revealed a re-formation of the cortical and medullary architecture.The interface be-tween reconstructed bone and the mandibular distal stump demonstrated osseous union with multiple mature bones and there was no evidence of fibrous union.The inferior alveolar artery and inferior alveolar nerve were through the midpor-tion of the regenerate bone after distraction period.The junc-tion between the transport disk and the proximal segment of mandible demonstrated osseous union of their interface.CONCLUSION This study demonstrates the ability feasibil-ity of using bifocal distraction osteogenesis to reconstruct mandibulectomy defects.This method may provide a satisfac-tory solution to a difficult problem in human mandibular re-construction. ......
