Acute hypervolume hemodilution for prevention of spinal hypotension during cesarean section

    ¡¾Abstract¡¿ AIM: To determine whether acute hypervolume hemodilution (AHH) with low molecule 6% hetastarch and Ringer¡¯s solution lowers the incidence of spinal hypotension during cesarean section (CS). METHODS: 52 ASA¢ñ-¢ò healthy parturients to have CS were randomly divided into two groups. Within 20 min preª²spinal anesthesia, those in group¢ñ(n=20) received 200 mL of Ringer¡¯s solution combined 500 mL of 6% low molecule hetasearch, and those in group¢ò (n=32) 500 mL of Ringer¡¯s solution. Spinal anesthesia was performed with 10 mg of 5 g/L bupivacaine hyperbaric soloution in all parturients. Hypotension lasting ¡Ý2 min was treated with IV bolous of ephedrine 10 mg and the neonatal Apgars scores were recorded. The incidences of spinal hypotension, nausea and vomiting and the use of ephedrine were compared between the two groups. RESULTS: The incidences of spinal hypotension, nausea and vomiting and event such as the administration of ephedrine in group¢òwere significantly lower than those in group¢ñ (P>0.01). The neonatal Apgar scores were high and similar in both groups (P>0.05). CONCLUSION: The AHH with 6% low molecule hetastarch and Ringer¡¯s solution can effectively prevent spinal hypotension during CS. ......
