⻊母外翻矫正术—3种手术方法分析

     摘要：[目的]观察利用三种手术方法治疗⻊母外翻畸形的疗效。[方法]自2000年1月~2003年12月实施⻊母外翻矫正术60例(83足)。术前按照生物力学原理对全部病例进行分析，根据⻊母外翻畸形程度、足部生物力学异常的状况和病人年龄分为3组。选择3种手术方法进行治疗：(1)第1跖骨头截骨术(Reverdin 手术)组：25例，39足。(2)软组织平衡手术(McBride手术)或结合第1跖骨基底截骨术组，15例，20足。(3)跖⻊母关节成形术(Keller手术)或结合第1跖骨基底截骨术组：20例，24足。[结果]本组60例手术全部成功，无手术并发症。48例得到随访，随访时间1～4 a，平均2 a 5个月。其中Reverdin手术组：20例，31足，优良率903%；McBride手术组：12例，16足，优良率875%；Keller手术组：16例，19足，优良率842%。[结论]⻊母外翻畸形是最常见的足部疾病，不同病人患足生物力学异常的状况是不同的。根据病人具体情况采取相应手术方法，恢复其正常的生物力学状态，才能获得良好的治疗效果。

    Hallux valgus deformity treated by three kinds of operations

    Abstract：[Objective]To observe the effect of three operative methods for treatment of hallux valgus deformity[Method]Sixty operations(83 feet)were performed on hallux valgus deformity cases from January 2000 to December 2003Before the operations,each individual case was carefully analyzed in compliance with the theory of biomechanics and devided into three different groups according to the gravity of the hallux valgus deformity,the abnormality of biomechanics of the concerned foot and the age of patientThe following three methods were chosen for the treatment:(1)The Reverdin operation:25 patients(39 feet);(2)The McBride operation(or in combination with osteotomy of the first metatarsal based):15 patients(20 feet);(3)The Keller operation(or in combination with osteotomy of the first metatarsal based):20 patients(24 feet)[Result]All of the 60 operations were successful with no postoperative complications,48 of these cases were closely followed up within a period of 1~4 years,averaging 2 years 5 months903% of the Reverdin operation(20 patients,31 feet)obtained excellent and good results;with 875% for the McBride operation(12 patients,16 feet)and 842% for the Keller operation(12 patients,16 feet)[Conclusion]Hallux Valgus is the most common foot diseaseThe abnormality of biomechanies of the foot varies from patient to patient,therefore different operative methods must be chosen for each individual case in order to regain the normal biomecbanics and attain the best possible results ......
