     Exchange transfusion with Rh+ blood for RhHDN: An analysis of 32 cases

    【Abstract】 AIM: To study the feasibility for maternal plasma exchange and for substitute of Rh (+) by Rh (-) blood in exchange transfusion for the treatment of RhHDN. METHODS: Enrolled in the study were 32 cases of RhHDN who had been diagnosed by both clinical symptoms and serum immunology in the lab (18 cases diagnosed during pregnancy and 14 cases after birth). The 18 cases were treated with maternal plasma exchange. Thirtyone of the 32 cases received exchange transfusion within 2472 h after birth (① case had only mild symptoms and did not receive exchange transfusion), of whom 6 were given Rh (-) blood and 25 cases used Rh (+) blood. RESULTS: Of the 18 cases treated with maternal plasma exchange, 16 cases had good results and 2 cases had no improvement. The 31 cases treated with exchange transfusion with Rh (+) blood or Rh (-) blood all had satisfactory results with no significant difference between them (P>0.05). All the 32 cases recovered. CONCLUSION: Maternal plasma exchange for Rh negative mother before or during pregnancy can alleviate fetus symptoms, help normal development and delivery and provide chances for exchange transfusion after birth to save the life of the newborns. Rh (+) blood can be used for exchange transfusion in RhHDN to block hemolysis, avoid nuclear jaundice and correct anemia. ......
