Orbscan Ⅱz对近视眼角膜形态的临床研究

     【摘要】 目的 用Orbscan Ⅱz裂隙扫描角膜地形图分析系统来研究近视眼的角膜形态,为近视眼角膜屈光手术提供临床指导。方法 检查353眼近视眼患者和381眼正常人的角膜前表面高度图、后表面高度图、后表面前突极值及分布,并进行对比分析。结果 近视眼各组角膜前表面高度图中则以不完全桥型为最多,各组所占的比率依次为48%、65%、60%、61%;近视眼的角膜后表面高度图均以岛型为主;近视眼的各组角膜后表面前突极值分别为:(27.66±8.31)μm、(29.0±9.62)μm、(26.88±9.10)μm、(29.90±9.51)μm,统计学分析各组近视眼之间差异均无显著性(P>0.05),各组近视眼的角膜后前突极点均多位于颞下象限、依次为鼻下象限、颞上象限、鼻上象限,随着近视的加深,其前突极点有向鼻下象限移动的趋向。结论 近视眼的角膜前表面高度图主要为不完全桥型,后表面高度图则主要为岛型;近视眼的屈光度与角膜后表面前突极值无相关性;近视眼的前突极点多位于颞下象限,随着近视的加深,有向鼻下转移倾向。

    Clinical study on corneal morphology in myopia using the Orbscan Ⅱz corneal topography system

    【Abstract】 Objective To investigate the corneal morphology in myopias with the Orbscan Ⅱz corneal topography system to supply more useful information for the refractive surgery.Methods 353 eyes of myopia 381 eyes normal were involved in this study.Anterior corneal surface elevation map,posterior corneal surface elevation map,value and location of posterior corneal protrusive pole, and corneal thickness were examined by the Orbscan Ⅱz corneal topography system.The results were analysed with SPSS10.0.Results In myopic eyes, incomplete ridge was the most common type,the rate of incomplete ridge in the different myopia groups were 48%,65%,60% and 61% respectively.For posterior corneal surface elevation map,island type was the most common pattern both in normal and myopic eyes.The value of posterior corneal protrusive pole was (28.05±10.05)μm in normanl eyes and (27.66±8.31)μm, (29.0±9.62)μm, (26.88±9.10)μm and (29.90±9.51)μm in the different groups of myopic eyes.There were significant differences between normal eyes and the different myopic group eyes(P>0.05).The posterior corneal protrusive pole both in normal and myopic eyes were mostly located at the inferotemporal quarter,followed by inferonasal,superotemporal and superonasal quarter,but,with the increase of myopia,it shifted to the inferonasal quarter.Conclusion In myoic eyes, the incomplete ridge was the most common pattern in the anterior corneal surface elevation map.There is no relationship between the refractive error and the value of the posterior corneal protrusive pole in myopic eyes. In myopic eyes,the posterior corneal protrusive pole is mostly located at the the the inferotemporal quarter,with the increase of myopia,it shifts to the inferonasal quarter. ......
