【摘要】 目的 评价和比较乳腺X线摄影、乳腺超声成像以及二者联合应用在乳腺疾病诊断中的准确率和应用价值。方法 2004年8月～2005年12月，对同时行乳腺X线检查和超声成像检查的120例患者135个病灶进行了病理检查。根据怀疑乳腺病变的恶性程度，将术前X线检查、超声成像、联合诊断的病变分别分为四级：Ⅰ级良性；Ⅱ级不能定性；Ⅲ级可能恶性；Ⅳ级恶性。前瞻性记录三种方式的分级，并与病理诊断对照。资料输入SPSS 11.0统计软件包，做相关统计学分析。结果 经病理检查诊断的135个乳腺病变中，良性病变89个，恶性病变46个。乳腺X线检查和超声成像的敏感性分别为89.1%和84.8%，特异性分别为88.8%和91.0%，诊断准确率均为88.9%。二者联合对乳腺恶性病变诊断的敏感性为97.8%，特异性为93.2%，诊断准确率为94.8%。二者联合的各项指标均优于单纯采用乳腺X线检查和超声成像，但无统计学差异。结论 乳腺X线摄影和超声成像诊断乳腺疾病各有优势，联合应用似可提高对乳腺病变的诊断准确率。

    【Abstract】 Objective This study was to evaluate the accuracy of ammography,ultrasonography and combination of both modalities for the diagnosis of breast lesions.Methods Between Aug 2004 and Dec 2005,135 breast lesions in 120 women diagnosed pathology were underwent mammography and ultrasonography.According to the grade of suspicion of malignancy by imaging anifestations,every lesion was prospectively classified by mammography,ultrasonography and combination of findings of both modalities into four categories as follows： category Ⅰ： benign,category Ⅱ： equivocal,category Ⅲ： probably malignant and category Ⅳ： malignant.The imaging classifications were correlated with pathology diagnosis.The statistic analysis of data was performed with SPSS 11 for Windows statistic software.Results Of the 135 breast lesions diagnosed pathology,89 lesions were proved benign and 46 were malignant.The sensitivity,specificity,and accuracy of mammography,ultrasonography were 89.1% and 84.8%,88.8% and 91%,88.9% and 88.9% respectively.The sensitivity,specificity,and accuracy of combination of the two modalities were 97.8%,93.2%,94.8%.Every index of the two modalities had significant increase than that of each single modalities.Conclusion A higher diagnostic value could be achieved by using combination of the two modalities. ......
