两种药物术后自控镇痛的临床观察

【摘要】 目的 比较曲马多、芬太尼与氟哌啶不同配伍术后患者自控镇痛(PCA)的临床效果。方法 70例(ASAⅡ～Ⅲ级)手术患者随机分为2组，T组(n=35)镇痛药液为曲马多+氟哌啶；F组(n=35)镇痛药液为芬太尼+氟哌啶。选用韩国Accufuser泵行PCA镇痛技术，采用双盲法对比观察。结果 两组患者静息时、活动时的镇痛效应、舒适评分、镇静评分差异均无显著性，恶心呕吐、皮肤瘙痒、呼吸抑制、头痛、幻觉、嗜睡等不良反应差异亦无显著性，患者总体满意度F组优良率为94%，T组优良率为74%。结论 术后行PCA镇痛时，曲马多+氟哌啶配伍镇痛效果与芬太尼+氟哌啶配伍同样有效，不良反应无明显差异，但由于临时用药方面存在的差别，芬太尼+氟哌啶组患者总体满意度明显高于曲马多+氟哌啶组。【关键词】 镇痛；患者自控镇痛；曲马多；芬太尼；镇痛药

【Abstract】 Objective To compare the efficacy and side effects with tramadol combined with droperidol and fentanyl combined with droperidol during patientcontrolled analgesia.Methods Seventy patients(ASAⅡⅢ)were randomly divided into two groups.Group T(n=35)received tramadol combined with droperidol;Group F(n=35)received fentanyl combined with droperid.With a Accufuser pump.Results There was no significant difference between the two groups in the pain scores and side effects.The F group has significantly higher scores than the T2 group in the overall satisfaction of pain relief.Conclusion Fentanyl group has no significant difference in the pain scores and side effects with tramadol group.But its has significantly higher scores in the overall satisfaction of pain relief. ......
