左心室腔动态重构与功能的二维超声新探讨

     【摘要】 目的 探讨正常大小的左心室几何形状及动态变化。方法 在随机收入的215例患者中,用二维超声测量左心室腔收缩及舒张末期长轴面积,观察其几何数学模式,分析左室动态重构形态改变并进行数学拟合。结果 (1)二维超声直接测得舒张末期左室腔面积值与用半椭圆几何公式计算值之间高度相关(r=0.79,P<0.001),均值分别为(24.45±4.75)cm2与(24.35±4.60)cm2,二者差异无显著性(t=0.83,P>0.40);(2)在收缩末期直接测得左室腔面积值与用三角形几何公式计算值之间高度相关(r=0.95,P<0.001),均值分别为(9.91±2.43)cm2及(9.95±2.31)cm2,二者差异无显著性(t=0.12,P>0.50)。结论 左心室内容积的几何形态随心肌舒缩而发生了重构,舒张末期心尖长轴切面接近半椭圆,容积则与几何半椭球相似,而收缩末期该平面更趋向三角形,容积则与几何圆锥接近。

    A novel method of exploring the left ventricular dynamical remodeling and function by two-dimensional echocardiography

    【Abstract】 Objective To explore the geometric patterns of the left ventricles(LV) dynamical remodeling in normal size.Methods Consecutively,in 215 patients using two-dimensional echocardiogram(2DE),the LV long-axis views in the end of systolic and diastolic periods were manually traced along the endocardial borders.At the same view,the authors compared the traced areas with that of the outcome collected by 2DE to measure the distances of the LV long and shot-axis,then calculated the geometric areas according to their shapes simulating as an semi-ellipse or a triangle.Results (1)There was close correlation between the traced areas and the outcomes from calculated by simulating as a semi-ellipse at the end of diastole(r=0.79,P<0.001);the average values of them were excellent correlation coefficients and low variability [(24.45±4.75)cm2∶(24.35±4.60)cm2;t=0.83,P>0.40];(2)Likely,there was much more excellent correlation between the traced areas and those calculated by simulating as a triangle at the end of systole(r=0.95,P<0.001)and the average values of them[(9.91±2.43)cm2∶(9.95±2.31)cm2;t=0.12,P>0.50)].Conclusion Because of the geometric remodeling of LV in the end of systolic and diastolic periods,the shape of LV cavity at the apical long axis in end of diastole is more close to be a semi-ellipse,and it tend to be a taper in the end of systole. ......
