伴腹膜后血肿的腹部/盆腔多脏器损伤65例诊治分析

     摘要：目的探讨伴腹膜后血肿的腹部/盆腔多脏器损伤的诊断与治疗。方法回顾性分析我院1996年1月～2005年3月收治的伴腹膜后血肿的腹盆部多脏器伤65例的临床资料。其中手术治疗51例(78.5%)，非手术治疗14例。手术中后腹膜切开探查29例(56.9%)，肝修补14例，脾切除或修补15例，胰体尾切除1例，胃修补3例，十二指肠修补及造瘘术11例，肠部分切除或修补术30例，肾修补13例，膀胱修补和造瘘术9例，血管修补5例。结果本组病例全部为多发伤，入院时休克41例(63.1%)。治愈58例，死亡7例，死亡率10.8%。死亡原因主要为失血性休克和多器官功能障碍综合征。结论伴腹膜后血肿的腹部/盆腔多脏器伤伤情复杂严重，死亡率高。早期诊断和抗休克治疗是救治成功的关键，并强调根据受伤部位和伤情的不同给予个体化的治疗方案。

    Abstract： Objective To investigate the diagnosis and treatment of patients with intraabdominal or pelvic viscera injuries complicated with retroperitoneal hematoma. Methods Clinical data of 65 cases of abdominal or pelvic multiple traumas with retroperitoneal hematoma admitted from January 1996 to march 2005 were studied retrospectively.
