摘要： 目的 探讨闭合性脾损伤非手术治疗的可行性及适应症。方法对1995年1月～2004年12月采用非手术治疗的78例闭合性脾损伤临床资料进行回顾性分析。结果 非手术治疗成功73例(93.6%)，住院时间12～30天。另5例因在非手术治疗期间由于血流动力学不稳定而中转手术治愈。结论 选择性非手术治疗脾损伤是安全、可行的，但必须严格掌握适应症和严密监测伤情变化，若在非手术治疗期间血流动力学不稳定或发生延迟性出血应及时手术治疗。关键词： 脾脏； 脾外伤； 非手术治疗
Nonoperative management for closed splenic injuries:a report of 78cases

Abstract： Objective To investigate the feasibility and indication of management for closed splenic injuries. Methods From January 1995 to December 2004,78 cases with closed splenic injuries were analyzed retrospectively in the clinical data. Results Seventy eight were cured. Among them,73 cases(93.6%)were cured by nonoperative management,5 cases were cured by operation because of unstable hemodynamics or delayed hemorrhage of patients during the period of nonoperative management. Conclusion Nonoperative management for closed splenic injuries is safe and feasible in a selected gloup of patients. But the indication must be strictly grasped and the change of traumatic status must be closely observed. The operation should be promptly performed once the patient's hemodynamics is unstable or delayed hemorrhage occurs during the period of nonoperative management. ......
