【摘要】 目的：研究急性脑卒中最佳治疗模式，对急性脑卒中患者的效果进行观察。方法： 共入选发病在7天以内的住院患者7 202例，2000年6月-2005年6月的患者，4 089例为治疗组，1995年5月-2000年5月的患者，3 113例为对照组(以往常规疗法)。治疗组采用急性脑卒中治疗模式，融神经内科、神经外科、康复科和神经介入科为一体的团队工作方式，对频繁发作TIA、进展型脑梗死、中、重型脑梗死、中大量脑出血和蛛网膜下腔出血的患者,实施多学科密切协作的医疗流程，以最佳医疗资源服务于患者，注重监护与早期康复介入。观察指标是出院时基本治愈率、总有效率、病死率、住院平均日、并发症和平均药费比，经统计分析该治疗模式的效果。结果：基本治愈率、总有效率、病死率、并发症、平均住院日和平均药品比,治疗组均优于对照组(P<0.001)。结论：急性脑卒中最佳治疗模式能降低患者的病死率，明显提高治愈率、降低致残率，提高回归社会的能力，并降低患者经济负担。

    Study of the best medical therapy on acute stroke

    【Abstract】 Objective：To study the best medical therapy for the patients with acute stroke. Methods:7202 patients in hospital were eligible for the criteria that the patients were admitted with acute stroke in 7 days .Of all patients, 4089 were made as treatment group (admission from June, 2000 to June, 2005); 3113 were made as control group (conventional therapy before June 2000). STMS acute stroke was used in the treatment group that includes the department of Neurology, Neurosurgery, Rehabilitation, and interventional Radiology. It was used in the patients with TIAs, Cerebral infarctions, Cerebral hemorrhage CH or Subarachnoid hemorrhage (SAH) so as to make those patients best services, monitoring, rehabilitation and interventional radiology. Parameters observed were at discharge curative rate, total effective rate, mortality; average in hospital days, complications, satisfied level and average ratio of treat-drug. All parameters were analyzed by SPSS software.Results: Curative rate, total effective rate, mortality, average in hospital days, complication, and average ratio of treat-drug were significantly better in the treatment group than in the control group (P﹤0.001). Satisfied level was not significant between two groups. Conclusions: STMS acute stroke may reduce the mortality of the patients with acute stroke, improve obviously curative rate, decrease the disability, increase the ability returning society, but don't increase patients, economic charge. ......
