双手微小切口白内障手术合并ThinOptX晶体植入术后的临床视觉效果分析

     【摘要】 目的 评估双手微小切口白内障超声乳化手术(bimanual ultrasmall-incision phacoemulsification)后,通过1.6mm透明角膜切口,植入 ThinOptX人工晶体植入术后的视觉及屈光效果。方法 本研究对40例植入ThinOptX人工晶体的病人(男18例,女22例)进行回顾性分析。所有的病人均通过1.4mm的角膜切口实施双手微小切口白内障超声乳化手术摘除白内障,并通过一个扩大1.6mm透明角膜切口,由特定的推注器植入不同度数的ThinOptX人工晶体。术后连续随访6个月,并对患者术前、术后第1周、第1个月、第3个月、第6个月的最佳矫正视力(BCVA)、屈光结果、眼压及手术并发症进行分析比较。结果 ThinOptX人工晶体通过1.6mm透明角膜切口全部顺利、安全的植入到晶体囊袋。术后病人的最佳矫正视力与术前相比有显著性的提高(P<0.0001),手术后第1个月80.0%的患者视力达到0.8,手术后散光与术前散光相比没有明显的统计学差异,眼压无明显变化,有3例病人发生术后晶体后囊膜混浊,但没有病人需要Nd:YAG治疗。结论 ThinOptX人工晶体使双手微小切口白内障超声乳化手术的优越性得以实现,但仍需完善。

    Visual outcome of bimanual phacoemulsification with implantation of an ThinOptX lens

    【Abstract】 Objective To assess the visual and refractive outcome of bimanual phacoemulsification with implantation of a ThinOptX intraocular lens through a microincision 1.6mm.Setting: Singapore National Eye Centre,Singapore.Methods In this consecutive retrospective noncomparative clinical trial,40 eyes were included(male 18 eyes,female 22 eyes).Bimanual phacoemulsification was performed through a 1.4mm corneal incision and ThinOptx IOL was inserted through an enlarged 1.6 incision.The patients refractive status and uncorrected and best corrected distance visual acuities,intraocular pressure,refractive results were assessed preoperatively and 1st week,1st month,3rd month and 6th month after surgery.The complications and rate of posterior capsule opacificaiton(PCO) were reported up to 6 months postoperatively.Results ThinOptX IOL intraocular lens were injected into capsular bags completely and safely.80.0% operated eyes of the final best corrected distance acuity were better than 6/7.5 1st month postoperatively.There are no significant difference of astigmatism between preoperation and postoperation.3 eyes had posterior capsular opacity postoperatively,but no eye had a neodymium:YAG laser capsulotomy.Conclusion Results show that ThinOptX IOL is a safe,effective lens that could be inserted through a 1.6mm incision,and provides excellent surgical performance,meanwhile itstill need to be improved. ......
