蛋白酶-抗蛋白酶失衡在兔肺挫伤中的作用

     摘要: 目的 研究蛋白酶-抗蛋白酶失衡在兔创伤性肺挫伤中的作用。方法 家兔随机分为2组,创伤组(n=8)和对照组(n=8),采用兔单侧肺挫伤生物撞击模型,动态观察血浆中性粒细胞弹性蛋白酶(NE)、α1抗胰蛋白酶(α1AT)、肿瘤坏死因子α(TNFα)及白介素8(IL8)含量变化;活杀动物后取肺组织比较湿/干重比值(W/D)、丙二醛(MDA)、髓过氧化物酶(MPO)的变化,观察支气管肺泡灌洗液(BALF)中上述细胞因子和炎症介质的变化,进行肺组织的病理学光镜检查。结果 创伤组与对照组相比较,血浆和BALF中NE的活性升高、血浆α1AT含量先降低后升高、TNFα、IL8含量均明显升高(P<0.01或P<0.05),兔肺组织损伤以肺出血、水肿和中性粒细胞(PMN)浸润为主要表现。结论 肺挫伤后,PMN激活并释放NE,使蛋白酶-抗蛋白酶失衡,在创伤性急性肺损伤中有重要的作用。

    Role of proteaseantiprotease imbalance in pulmonary contusion in rabbits

    Abstract: Objective To investigate the role of proteaseantiprotease imbalance in pulmonary contusion after the chest impact trauma in the rabbits.Methods Rabbits were randomly divided into two groups: traumatic group (n=10) and the normal control group (infusion of saline,n=8).The isolated lung contusion model of the rabbit was established,induced by a bioimpact machine.The plasma concentrations of elastase and α1antitrypsin (α1AT) were determined.The changes in tumor necrosis factorA(TNFα) and interleukin8(IL8) contents were measured both in plasma and bronchalvelor lavage fluid (BALF).The contents of malondialdehyde (MDA) and myeloperoxidase (MPO) were detected in the lung tissue.The wet/dry weight ratio,the contents albumin in BALF and the histological features of lung were examined.Results Compared with the control group,the activity of NE,TNFα and IL8 was increased both in plasma and BALF,but the concentrations of α1AT were reduced initially,and then elevated later in plasm.The contents of MDA and MPO were raised in the lung tissue.The ratio of W/D and albumin in BALF were higher than that of the control group(P<0.01 or P<0.05).Pathological examination revealed marked changes ......
