【摘要】 目的 探讨脾结核的CT诊断与鉴别诊断。方法 回顾性分析7例经手术及病理证实的脾结核CT表现。结果 本组病例表现为脾脏增大，6例平扫见多发大小不等结节状及斑片状低密度病灶，边界欠清，增强后病灶内无强化，边界变清楚，1例除脾内较大斑片状低密度灶外，还可见小斑片状高密度影(合并出血)，1例为散在小点状钙化灶，4例伴有其他脏器结核。结论 脾脏多发大小不等低密度无强化病灶是脾结核的主要征象，但不具有特异性，而淋巴结结核的CT表现具有特征性，发现脾外结核的证据能确立诊断，CT是发现脾结核较好的检查方法。【关键词】 脾结核；鉴别诊断；体层摄影术，X线计算机

【Abstract】 Objective To explore CT diagnosis and differential diagnosis of splenic tuberculosis.Methods The CT findings of 7 cases with splenic tuberculosis confirmed by operation and pathology were analyzed retrospectively.Results The CT findings were enlargement of spleen,multi-nodular hypodensity lesions were found in 6 cases with boundary blurred.There was no enhancement in the lesions,but boundary became clear.Small patch hyperdensity shadow (hemorrhage) was found in one case besides great patch hypodensity lesions;sparse tiny calcified lesions in one case and other organs tuberculosis in 4 cases.Conclusion The main presentation of CT for splenic tuberculosis is multiple,different sizes of calcifi-cation with hypodensity and no enhancement,but it is not specific.The CT findings of lymph node tuberculosis are characteristic,the findings of extra-splenic tuberculosis will support a diagnosis of splenic tuberculosis.CT is a feasible method to find and diagnose splenic tuberculosis. ......
