踝关节骨折手术疗效观察

【摘要】 目的 总结治疗踝关节骨折的手术方法和治疗效果。方法 自2002年8月～2004年8月，手术治疗踝关节骨折120例并对其进行分析。男64例，女56例；根据Weber-AO分类B型62例，C型58例。应用改良的Baird和Jackson评分系统对其中疗效进行评定，并将骨折类型、骨折固定方法、术后功能锻炼的关系进行分析。结果 随访时间为8～36个月，平均18个月。120例疗效为：优72例，良30例，可12例，差6例，优良率85%。除8例长时间行走有轻微疼痛外，其余患者均无疼痛。结论 手术治疗踝关节骨折可取得满意的骨折复位和临床疗效，复位的质量与疗效密切相关，判断骨折类型和选择固定方法对于提高复位质量，改善远期疗效十分重要。【关键词】 踝关节；骨折固定术，内；治疗结果

【Abstract】 Objective To evaluate the surgical techniques and results of open reduction and internal fixation performed for ankle fracture.Methods Eightytwo patients with ankle fracture were treated with open reduction and internal fixation from August 2002 to August 2004.One hundred and twenty patients with complete clinical date were analyzed. The patient group consisted of 64 males and 56 females.According to WeberAO classification,the group was made up of 62 Weber B cases,58 Weber C cases.All patients were evaluated with modified Baird and Jackson scoring system.The relationship between final result and fracture pattern,fixation methods,the time of exercise initiation after operation were analyzed respectively.Results The followup period varied from 8 months to 36 months, with an average of 18 months.The number of patients whose results was excellent,good,fair and poor was respectively 70,32,12 and 6.The total percentage of good to excellent clinical results was 84.5%.Fiftyfour ankles were completely pain free and the remainders had only slight pain after long time walking.Conclusion Operative treatment may provide satisfactory fracture reduction and clinical results for ankle fracture.Correct fracture pattern estimation and people internal fixation is of importance to achieve and maintain reduction,and then,to gain better long term results. ......
