血管紧张素转换酶基因多态性与脑卒中伴发高血压的关系

     摘 要 目的:研究血管紧张素转换酶(ACE)基因多态性和脑卒中伴发高血压的发病关系。方法:应用PCR方法检测105例脑卒中患者(其中伴发原发性高血压76例)、106例原发性高血压(EHT)患者及64例正常人的ACE基因的缺失/插入(D/I)多态性,同时测定血浆血管紧张素Ⅱ(AngⅡ)水平。结果:脑卒中组ACE基因型及等位基因频率与EHT组及正常对照组比较差异无显著性意义。脑卒中伴EHT组与无伴EHT组比较,Ⅱ基因型频率明显升高,而DD基因型频率则明显降低。脑卒中伴EHT组Ⅱ基因型频率高于EHT组,而DD基因型频率远低于EHT组和正常对照组,同时脑卒中伴EHT组的平均年龄较其他组高。脑卒中组和脑卒中伴EHT组Ⅱ基因型患者的AngⅡ水平均比DD基因型患者高。结论:原发性高血压患者中Ⅱ基因型携带者可能具有卒中易感性,原因可能与其AngⅡ水平较高有关。

    Relationship between angiotensinconverting enzyme gene polymorphism and stroke with hypertension

    Abstract Objective: To investigate the relationship between the polymorphism of the angiotensin converting enzyme(ACE)gene and the incidence of stroke with hypertension. Methods: An insertion/deletion polymorphism of the ACE gene was analysed by polymerase chain reaction in 105 cases of stroke(76of them accompanied by essential hypertension), 106 cases of essential hypertension(EHT) and 64 control subjects. Their angiotensin Ⅱ in blood were also determined. Results: No difference in genotype and allele gene frequency of ACE was observed between the patients with stroke and the control subjects or between EHT patients and the control subjects(p>0.05). The frequency of the Ⅱ genotype of the ACE gene was significantly higher in stroke patients with EHT than in those without EHT(χ2 =4.08, P<0 .05), yet the fequency of the DD genotype was on the contrary (χ2 =8.85, P<0.01). The frequency of the Ⅱ genotype of the ACE gene was significantly higher in stroke patients with EHT than in EHT patients without stroke(χ2=7.24, P<0.01) , yet the frequency of the DD genotype was obviously lower in stroke patients with EHT than that of the EHT patients and the control subjects(χ2=5.30 and 4.69, P<0.05). The average age of the stroke patients with EHT was significantly higher than that of the EHT patients and the control subjects(t=4.62 and 7.07 ,P<0.001).The levels of the plasma angiotensinⅡwere obviously higher in patients with Ⅱ genotype than those with DD genotype in stroke patients and stroke plus EHT patients. Conclusion: These results suggested that the Ⅱ genotype of ACE gene increased the risk of stoke in EHT patients. The reason may be related to the higher level of angiotensin Ⅱ. ......
