【摘要】 目的 探讨BiPAP无创通气治疗在重症肺结核合并Ⅱ型呼衰的治疗作用。方法 对24例重症肺结核合并Ⅱ型呼衰患者采用BiPAP呼吸机经鼻(面)罩双水平气道正压通气，观察治疗前后血气分析的pH、PaO2、PaCO2、SaO2指标变化。结果 BiPAP无创通气治疗后PaO2、SaO2均明显升高，PaCO2明显降低，pH明显改善，差异均有非常显著性(P<0.01)。结论 BiPAP呼吸机经(鼻)面罩双水平气道正压通气治疗重症肺结核合并Ⅱ型呼衰疗效满意，无明显并发症，值得临床推广应用。

    【Abstract】 Objective To investigate the therapeutical effect of bi-level positive airway pressure (BiPAP) noninvasive ventilation in the severe pulmonary tuberculosis with typeⅡrespiratory failure.Methods Before and after utilization of BiPAP ventilator，the blood gas parameters including pH,PaO2,PaCO2,SaO2,were observed in the mode of positive airway pressure ventilation via nasal and facial mask in 24 patients with severe pulmonary tuberculosis with typeⅡrespiratory failure.Results Both of the values of PaO2 and SaO2 increased obviously,while the value of PaCO2 decreased obviously,and the value of pH improved significantly after utilization of BiPAP noninvasive ventilation.All the differences were statistically significant (P<0.01).Conclusion Utilization of BiPAP ventilator via nasal and facial mask positive airway pressure ventilation in the severe pulmonary tuberculosis with typeⅡrespiratory failure has a satisfactory therapeutic effect,without any obvious complications and deserves further clinical application. ......
