经直肠双平面超声在直肠癌术前分期中的应用价值

     摘要: 目的: 评价经直肠双平面超声在直肠癌术前分期中的应用价值。方法: 对47例经活检证实为直肠癌的患者术前应用双平面腔内探头行经直肠超声(TRUS)，将超声分期与手术病理分期进行对照分析。结果: 34例直肠癌术前超声分期与手术病理分期一致，准确度为72%，8例过度分期，5例分期不足。超声诊断淋巴结转移的准确度为60%，灵敏度为41%，特异度为76%。结论: 经直肠双平面超声对直肠癌术前分期具有较高的应用价值，但对肠周转移淋巴结的检出仍有一定局限性。

    The use of doubleplane transrectal ultrasonography in the preoperative staging of rectal cancer

    Abstract: Objective: To evaluate the accuracy of the use of doubleplane transrectal ultrasonography (TRUS) for preoperative staging of rectal cancer. Methods: Fortyseven patients proved rectal cancer by biopsy were examined and staged preoperatively with TRUS. The pararectal nodes were classified as malignant on the basis of echogenicity. Compared to pathologic staging, the accuracy, sensitivity and specificity were calculated. Results: Thirtyfour masses were staged correctly by TRUS: 3UT1, 5UT2, 25UT3, 1UT4. Eight were overstaged and five understaged. The overall staging accuracy was 72%. Ten patients who having ambulant nodes by TURS and seventeen patients without lymph nodes metastases were confirmed by histopathology. The accuracy was 60%, the sensitivity was 41%, the specificity was 76%. Conclusions: It was indicated that doubleplane TRUS is an effective way to detect the depth of tumor infiltration. The detection of pararectal nodes metastases is still a limitation. ......
