激素替代疗法对绝经后妇女乳腺的影响

     摘要: 目的：探讨激素替代疗法(HRT)对绝经后妇女乳腺的影响。方法：90例绝经后有更年期综合征症状的健康妇女分为2组，治疗组48例，给予结合雌激素片剂0.625 mg+安宫黄体酮4 mg(周期方案);对照组42例，给予谷维素10 mg，每日2次，连续服药12个疗程。观察2组用药前、后双侧乳腺组织红外线扫描、Kupperman评分、血清生化指标、子宫内膜厚度等的变化。结果：与对照组用药后及治疗组用药前相比，治疗组用药后近红外线扫描乳腺密度及血管发生的变化差异有统计学意义(P<0.05)， Kupperman评分差异亦有统计学意义(P<0.05),甘油三酯(TG)、低密度脂蛋白(LLDL)水平明显降低，高密度脂蛋白(HLDL)明显升高，差异有统计学意义(P<0.05)；治疗组用药前、后子宫内膜厚度分别为(3.21±1.12)、(4.67±2.01)mm，差异有统计学意义(P<0.05)。子宫内膜厚度>5 mm的患者宫腔镜检查及内膜活检均在正常范围。结论：HRT用于治疗绝经后更年期综合征妇女时会发生乳腺结构改变,但此改变是可逆的。规范化监测及适当的疗程可降低HRT的风险。

    A study on the effect of HRT to the breast in postmenopausal women

    Abstract： Objective： To appoarch the effect of hormone replacement therapy (HRT) on the breast in postmenopausal women. Metheods： Ninety health women with postmenopausal syndrome were divided into two groupes ,48 cases received HRT 12 months .Compared with 42 health women with postmenopausal syndrome in the same time.To observe the scan breast in near infrared ray; Kupperman index； B ultrasonogrphy measure the thickness of endometrium et al. Results： Fourtyeight cases in HRT therapy scaned breast in near infrared ray were different from that have not HRT cases in structure and denisty(P<0.05); Kupperman index were differ from the contrast group(P<0.05); TG, HLDL, LLDL were also different from the control group; B ultrasonogrphy measure the thickness of endometrium increased(P<0.05) ......
