【摘要】 目的 探讨缺血性脑卒中的多层螺旋CT灌注成像诊断与表现。方法 12例脑卒中患者行了脑4层螺旋CT灌注成像(CTP)检查，在工作站生成脑血流量(BF)、血容量(BV)、平均通过时间(MTT)等局部脑组织血流动力学参数色阶图，并测量兴趣区这些参数值，以t检验比较分析。11例于12h至5天内作了CT复诊。结果 动态CTP显示了11例局部脑组织低灌注区和1例可疑病灶，主要表现为相应区域脑组织BF、BV数值下降、MTT数值增加及其彩阶图异常，12例患者均出现了局部脑组织MTT延长，其中BF、BV、MTT均异常者6例，局部脑组织BF、BV、MTT彩阶图异常范围大于实际缺血区。梗死或可疑梗死灶中心、周边区及对侧相应区域测量的BF、BV、MTT平均值差异有显著性(P<0.05)。结论 多层螺旋CTP对急性缺血性脑卒中有早期诊断价值，其中MTT是缺血性脑卒中反应最为敏感的功能与形态学定量分析指标。

    Multislice CT perfusion diagnosis of acute ischemic stroke

    【Abstract】 Objective To discuss the diagnostic value of MSCT perfusion imaging on acute ischemic stroke.Methods Twelve patients with acute ischemic stroke were underwent brain 4-slice CT using routine and perfusion imaging techniques.CT perfusion mapping and quantitative measurement programs were then analyzed and computed at workstation related parameters of cerebral hemodynamics on dynamic CT images，including cerebral blood flow (BF)，cerebral blood volume (BV) and mean transit time (MTT) of ROI.The CT follow-up in 11 cases after 12 hours to five days.Results Regional cerebral hypoperfusion in 11 cases while just slightly delayed MTT maps in one case were detected on dynamic CT perfusion imaging.It mainly showed the value of regional cerebral BF and BV decreased ......
