腰椎滑脱中棘突阶梯征的临床意义

     摘要: 目的：探讨棘突阶梯征在腰椎真性和假性滑脱时的不同表现及其诊断价值。方法：对82例腰椎滑脱患者的腰椎侧位片进行分析，其中50例为腰椎滑脱组，32例作为对照组，对每一张侧位片中L34和L45棘突间的阶梯征进行评价并记分。结果：真性腰椎滑脱时棘突阶梯征位于滑脱节段上方水平，评价值为2.96；假性滑脱时棘突阶梯征位于滑脱节段下方水平，评价值为2.61,评价值均高于对照组的0.81(P<0.01)。结论：棘突阶梯征对真性和假性腰椎滑脱具有鉴别诊断意义。

    The diagnostic significance of spinous process step sign in lumbar spondylolisthesis

    Abstract: Objective: To discuss the difference and its diagnostic value of spinous process step sign in true and pseudolumbar spondylolisthesis. Methods: Eightytwo lumbar lateral films were reviewed, 50 of which with the diagnosis of spondylolisthesis and 32 of which as controls. The spinous process step sign at every L34 and L45 segments was scored. Results: In true spondylolisthesis the spinous process step locates above the level of vertebral body slippage, which score 2.96; whereas in pseudospondylolisthesis the step locates below the level of slippage, which score 2.61. Both scores are higher than controls (0.81), P<0.01. Conclusion: Spinous process step sign can help to differentiate true and pseudospondylolisthesis. ......
