路植骨融合椎弓根螺钉固定治疗32例中老年腰椎管狭窄并腰椎不稳疗效分析

     摘要: 目的：探讨后路植骨融合椎弓根螺钉固定治疗中老年腰椎管狭窄合并腰椎不稳的疗效及术式选择适应证。方法：对32例中老年腰椎管狭窄合并腰椎不稳经过严格保守治疗无效的患者采用开窗椎间盘切除椎管减压和后路椎间、椎板、横突植骨融合及MossMiami椎弓根螺钉系统固定进行治疗。结果：术后随访9个月～3年，患者症状明显改善，融合率达96%，优良率达97%，未见假关节形成,无感染松动及断钉发生。结论：对于中老年腰椎管狭窄合并腰椎不稳患者根据症状、体征、影像学检查选择后路融合方法并给予MossMiami系统固定融合率高，手术彻底，临床疗效良好。

    Bone transplantation, intervertebral fixation system for treatment of the elderly lumbar canal narrowness combined with instability

    Abstract: Objective: To investigate the surgical treatment method and effect of lumbar canal narrowness combined with instability of elderly. Methods: Surgical operations has been performed for 32 elder patients with lumbar canal narrowness combined with instability after strict conservative treatment failure. Adopting expletory laminetomy with a small window and intervertebral discs were picked out, spinal canal pressure reduced, self granule bone transplanted to vertebral fusion and fixed using MossMiami system. Results: Patients were followed up for 9 months to 3 years after surgery, symptoms obviously decreased, fusion effective rate 96%, excellent rate 97%, no nails break or regress. Conclusions: The MossMiami system is very effective for elders with lumbar canal narrowness combined with instability selected for surgery depending on symptoms,signs and presentation of films of Xray, CT and MRI. ......
