【摘要】 目的 分析纳科思联合顺铂胸腔注射治疗恶性胸腔积液的临床疗效。方法 23例患者引流尽胸水后胸腔内注入纳科思1500万U+顺铂60 mg+生理盐水50 ml后闭管，闭管2 h后开放引流，并观察治疗效果，效果不佳者1周后重复上述治疗，最多3次用药。结果 患者治疗总有效率达86.95%，其副作用发热和胸痛均较轻，患者能耐受，且无严重骨髓抑制，肝、肾功能损害。结论 胸腔内应用纳科思联合顺铂治疗恶性胸腔积液具有疗效好、不良反应小、安全性高的特点，值得在临床推广应用。

    【Abstract】 Objective To analysis clinical therapeutic effect of combination of rmhTNF and cisplatin in the patients with malignant pleural effusion.Methods 23 patients histologically proved malignant pleural effusion were injected with drugs (rmhTNF 1.5×107IU+cisplatin 60 mg+normal sodium 50 ml) into pleural via the catheter after having been drained away the pleural effusion,then clamped the catheter about 2 hours, and observed the therapeutic efficacy.The patients of ineffective repeated the drugs above mentioned after 1 week, 3 times at most.Results The total efficacy rate was 86.95%.The side effects of fever and chest pain are less, and the patients could tolerate them. No Serious myelosuppression and liver, kidney da mage was found.Conclusion Pleural perfusion with rmhTNF and cisplatin was effective,less adverse reaction and safe in treatment of malignant pleural effusion. So it deserves clinical popularizing. ......
