牙种植体龈沟液中骨钙素水平检测分析

     摘要: 目的: 研究牙种植体周围组织不同状态与牙种植体周围龈沟液(PISF)中骨钙素(OC)的关系。方法:根据临床及影像学标准,将30例51枚牙种植体分为牙种植体健康(HI)组、牙种植体龈缘炎(GI)组和牙种植体周围炎(PI)组,考察牙种植体周袋深度(PPD)、改良菌斑指数(mPLI)、改良龈沟出血指数(mBI)和骨丧失(BL)情况,用放射免疫分析法检测PISF中OC水平。结果:HI组、GI组及PI组牙种植体PISF中OC水平分别为(228.00±161.42)ng/ml、(241.48±141.09)ng/ml、(572.98±161.88)ng/ml,3组的OC水平差异有统计学意义(P<0.05),其中PI组OC水平大于HI组和GI组,差异有统计学意义(P<0.05),HI组和GI组之间差异无统计学意义(P>0.05)。OC水平同PPD、mBI、mPLI、BL均呈显著正相关,分别为r=0.68,P<0.01;r=0.45,P<0.01;r=0.34,P<0.05;r=0.62,P<0.01。以OC水平≥410 ng/ml作为诊断牙种植体周围炎的诊断截割点时,诊断实验的灵敏度为85%,特异度为78%,阳性预报值为60%,阴性预报值为93%。结论:PISF中OC水平可以在一定程度上反映牙种植体周围组织的炎症状况及骨组织的变化,并且有可能作为预测牙种植体周围炎的指标。

    Analysis of osteocalcin in the periimplant sulcular fluid

    Abstract: Objective: To investigated the levels of osteocalcin (OC) in the periimplant sulcular fluid (PISF) from dental implants. Methods: All 51 titanium implants were divided into three groups, 21 healthy implants(HI),16 gingivalis implants (GI) and 14 periimplantitis implants(PI) associated with traditional periobontal clinical parameters around dental implants. OC was detected by radio immunoassay. Results: The levels of OC were 228.00±161.42 ng/ml in HI ; 241.48±141.09 ng/ml in GI; 572.98±161.88 ng/ml in PI. The difference among HI, GI and PI was statistically significant at P<0.05 level. Significant difference existed between HI/PI and GI/PI. No statistically significant difference was found between HI and GI. The levels of OC in PISF were correlated with clinical parameters. (with PPD r=0.68, P<0.01; with mBI r=0.45, P<0.01; with mPLI r=0.337 P<0.05; with BL r=0.619, P<0.01). When the threshold for a positive level of OC test was set 410 ng/ml, the sensitivity =85%; the specificity =78% were found in the detection of periimplantitis. The positive predictive value was 60% and the negative predictive value was 93%. Conclusion: The levels of osteocalcin in PISF could be used to evaluate the healthy condition of the periimplaqnts. ......
