毛细血管全血筛查高血糖和高血脂效度的研究*

     【摘要】 目的 探讨Accutrend GCT检测仪用于流行病学现场调查中筛查高血糖、高血脂的可行性、筛查效度以及末梢毛细血管全血葡萄糖、胆固醇、甘油三酯的筛查切点。方法 采集51名自愿受试者空腹指末梢全血和静脉血,用Accutrend GCT检测仪测定末梢全血葡萄糖(CG)、胆固醇(CTC)和甘油三酯(CTG),用日立7060C全自动生化检测仪测定血浆葡萄糖(PG),血清TC和TG。结果 Accutred GCT检测仪检测的CG、CTC、CTG测定值与自动生化仪检测的PG、血清TC、TG测定值之间呈显著相关,控制受试者性别、年龄后的偏相关系数分别为0.93(P<0.001)、0.84(P<0.001)、0.63(P<0.001),CG、CTC平均测定值低于PG、血清TC,相对误差分别为-10.96%和-1.74%。CTG平均水平显著高于血清TG水平,相对误差为+46.41%。以自动生化仪检测的PG水平为6.1mmol/L、血清TC为5.2mmol/L、血清TG为1.7mmol/L分别作为诊断高血糖、高胆固醇、高甘油三酯的标准,当Accutrend GCT检测仪检测的CG、CTC、CTG分别为5.4 mmol/L、4.6mmol/L、1.5mmol/L时,其各自的筛查效度较高。结论 Accutrend GCT检测仪具有快速、简便、针刺痛感小、准确性和重复性好、便于携带的优点,与全自动生化仪检测的血浆葡萄糖、血清胆固醇和甘油三酯结果相关性较好,可用做以人群为目标的流行病学现场调查中血糖和血脂高危人群的筛查工具。

    Evaluation on the validity of screening for hyperglycemia and hyperlipidemia using finger-stick capillary whole blood sample

    【Abstract】 Objective To explore the sensitivity and specificity of screening for hyperglycemia and hyperlipidemia individuals using finger-stick capillary whole blood by Accutrend GCT measuring system. Methods Finger-stick capillary whole blood and venous blood samples were token after overnight fasting from 51 volunteers, capillary whole blood glucose (CG)、total cholesterol (CTC) and triglyceride (CTG) were measured by Accutrend GCT measuring system. Plasma glucose (PG), serum total cholesterol (TC) and triglyceride (TG) were measured by HITACHI 7060C autobiochemistry analyzer. Results The concentration of CG, CTC and CTG were significantly correlated to the levels of PG, serum TC and TG. The partial correlation coefficient after taking account of subjects gender and age were 0.93(P<0.001), 0.84(P<0.001)and 0.63(P<0.001), respectively. The mean levels of CG and CTC were lower than the levels of PG and serum TC giving the relative errors of-10.96%and -1.74%, respectively. CTG level was higher than the serum TG level and had a higher relative errors of +46.41% compare to the serum TG level. Using the levels of 6.1 mmol/L, 5.2mmol/L and 1.7mmol/L for PG, serum TC and TG, respectively as the standard for identifying the individuals with hyperglycemia, hypercholesterolemia and hypertriglyceridemia, the sensitivity and specificity were 100% and 91.7% at level of CG=5.4mmol/L, 100% and 85.1% at CTC=4.6mmol/L, 85.7% and 75.0% at CTG=1.7mmol/L, with each own relative higher accuracy for screening. Conclusion Accutrend GCT measuring system is featured with quick, simple, low pricking feeling, portable, higher accuracy and reliability, and can be used as the screening application mean to identify risk individuals with hyperglycemia and hyperlipidemia population-based study on cardiovascular risk factors. ......
