文拉法辛缓释剂与SSRTs治疗难治性抑郁症对照研究

     【摘要】 目的 探讨文拉法辛缓释剂治疗难治性抑郁症的临床疗效及安全性。 方法 将60例难治性抑郁症患者随机分为两组各30例,分别给予文拉法辛缓释剂和SSRTs抗抑郁剂治疗,疗程12 mo。于治疗前及治疗1 mo、3 mo、6 mo、12 mo末采用汉密顿抑郁量表评定临床疗效,副反应量表评定不良反应,生存质量量表评定生存质量。结果 汉密顿抑郁量表评分两组治疗1 mo末起均较治疗前有显著或极显著下降(P<0.05或0.01),两组间同期评分比较,研究组较对照组下降更显著(P<0.05)。生存质量量表评分两组治疗1 mo末起均较治疗前有显著或极显著升高(P<0.05或0.01),两组间同期评分比较,研究组心理领域、社会关系、环境因素因子分较对照组升高更显著(P<0.05或0.01)。两组不良反应均较轻微,除口干、多汗、便秘发生率两组有显著性差异(χ2=4.94,3.92,5.12,P<0.05)外,余差异无显著性。 结论 文拉法辛缓释剂治疗难治性抑郁症疗效较SSRTs显著,且能显著改善患者的生存质量,安全性高,依从性好。

    Control study of venlafaxine and SSRIs in the treatment of refractory depression

    【Abstract】 Objective To explore the clinical curative effect and safety of venlafaxine in refractory depression. Methods 60 patients with refractory depression were randomly divided into study (venlafaxine) and control group(SSRIs) for 12 months. Before treatment and at the ends of 1st, 3rd, 6th and 12th month of treatment, the clinical curative effects were assessed with the Hamilton Depression Scale (HAMD), adverse effects with the Treatment Emergent Symptom Scale(TESS) and quality of life with the WHO Quality of Life(WHOQOL). Results At the end of 1st month , scores of the HAMD decreased significantly or very significantly for both the two groups compared with pretreatment(P<0.05 or 0.01),in corresponding period those of the research group did more significantly compared with the control group(P<0.05). At the end of 1st month, scores of the WHOQOL increased significantly or very significantly for both the two groups compared with pretreatment(P<0.05 or 0.01), in corresponding period scores of psycho domain, social relationship and environmental factor did more significantly in the research group compared with the control group(P<0.05 or 0.01).The adverse effects of both groups were mild, and there were significant differences in dry mouth, hyperhidrosis, constipation between the two groups(P<0.05). Conclusion Venlafaxine is better than SSRIs in the treatment of depression, can notably improves patients’ quality of life, and has higher safety and better compliance. ......
