    Abstract:Objectives:To improve the nutritional situation an d increase the cure rate in patients with critical diseases by parenteral nutrit ional support of total nutrient admixture(TNA). Methods:TNA wer e administered in 84 patients.The duration of TNA-nutritional support was 5~61 d ays,with a mean of 20.5 days.After nutritional support,the nutritional status wa s improved in 13 malnurished patients with colon cancer and the operations were completed with no complications.In mechanically ventilated 36 patients the nutri tional situation were improved and 31 patients were smoothly free of ventilator. Conclusions:The parenteral nutrition of total nutrient admixtu re is an important measure in the treatment of critical patients.

    Continuous observation for the composition of human body

    in hyperthyroidism patients in the period of perioperation

    Abstract:Objectives:To explore the chagnes in nutir tional situation of the patients with hyperthyroidism at admission and in the pe r iod of perioperation with (BFA-100). Methods:Indeses (WT/HT,TS F,A MC,GS,BF) were conducted in 85 patients with hyperthyroidism within 3 day of adm ission.Nutritional situation was continuously evaluated in 54 patients 1 day bef ore and 5 day after operation with same method. Results:Malnutr ition in different degrees were found to be 96%(82/85) at admission.Wit h preo perative preparation, WT/HT,TSF,BF were significantly higher at the day before o p eration than those at admission.No significant changes were found after operation. Conclusions:Malnutrition of the patients with hyperthyroidism could be determined by the measurement of composition of human body with bioelectricity impedance.

    Nutritional support in patients with chronic heart dysfunction

    Abstract: Objectives:To observe the role of parenteral nutrition in patients with chronic heart dysfunction. Methods:36 cases of patients were divided into two groups.All patients rec eived the regular therapy against heart dysfunction.Of them,20 cases were supported wi th parenteral nutrition.The other patients were as controls.The clinical effecti veness and indexes were compared between the two groups before and after the tre atment. Results:The rate of clinical improvement was 85% in parenteral nutrition g roup,s ignificantly different from that in the control group(50%).The SV,CO and EF of l eft heart were obviously increased after PN. Conclusions:Nutritional support is helpful to improve the heart function in chronic heart dysfunction pateints.

    Effect of 11.4%Novamin on protein metabolism in postoperative patients

    Abstract:Objectives:The study was designed to obser ve the effect of 11.4%Novamin on protein metabolism in postoperative patients. Methods:70 cases of patients were randomly divided into two gr oups and were supported with total parenteral nutrition for 6 days after their a bdominal surgery.105 kJ(25 kcal)/(kg.d) of nonprotein calorie and 0.15 g/(kg.d ) of nitrogen were provided in every patient. 11.4%Novamin was used in one group a nd 7%Vamin was used in another group as control.Body weight,total protein and al bumin concentrations and plasma amino acid profile were measured before and afte r the study.Daily nitrogen output in urine and other drainages was determined an d nitrogen balance was calculated during the TPN period. Results:The differences in body weight,total protein and albumin concentrations and am ino acid profile were not significant between the two groups.The difference in a ccumulated nitrogen balance was significant,(-82.2±24.7)mg/kg body weight in 11 .4%Novamin group and (-171.1±201.6)mg/kg body weight in 7%Vamin group(P<0.0 5 ). Conclusions:The use of 11.4%Novamin in postoperative patient s is helpful to further improve the nitrogen balance when compared with 7%Vamin.

    The research of total parenteral nutrition plus recom binant human growth hormone in patients following major abdominal surgery

    Abstract:Objectives:To research the effects of usin g recombinant human growth hormone(rhGH) with total parenteral nutrition(TPN) on nitrogen balance and nutritional state in patients following major abdominal sur gery. Methods:We randomly selected 27 patients receiving TPN after major abdominal surgery and distributed them to study group(rhGH+TPN,n =15) and control group(TPN only,n=12).After operation,every one was g iven rhGH 4U or placebo by hyperdermic injection for 7 days. Results:The resul ts showed that TPN+rhGH increased the nitrogen balance,the level of plasma album in and transferrin,the body weight and creatinin height index(CHI).But the thick ness of triceps skin fold(TSF) had no significant change. Conclusions: The rhGH can improve the effects of TPN.

    Abstract:Objectives:To study the effect of Intralip id on the release of oxygen radicals from phagocytes in vitro. Meth ods: Human polymorphonuclear neutrophils(PMN) were purified and incubated eit her with purified lipopolysaccharides(LPS) or with Intralipid.The release of oxy gen radicals was measured by the nitroblue tetrazolium(NBT) test. Resul ts:The incubation of PMN with LPS increased the release of oxygen radica ls.However,when the PMN were incubated with LPS together with Intralipid,the rel ease of oxygen radicals was significantly decreased. Conclusions:Intralipid might inhibit the effect of LPS on the PMN in an infection with Gra m-negtive bacteria.

    Nutritional support for patient will liver transplantation

    Abstract:Objectives:To review the methods of nutrit ional support after liver transplantation. Methods:An analysis of the nutritional support and of the nutritional status in two p atients was made af ter orthotopic liver tranplantation(OLT).Total parenteral nutrition(TPN) was ad minis tered within first two or three days after the operation.The combination of enteric nutrition(EN) and parentera l nutrition(PN) was adopted three or four days after the OLT,and then a gradual intake of oral food was used. Results:Both patients surv ived with good health. Conclusions:Depending on the recovery of the transplanted liver functions,the supply of nutrition should be controlled d uring the f irst few days,and then gradually increased.Once the gastroenteric functions of p atients recov er,it is advisable to start EN as soon as possible.

    Influence of short chain fatty acids(SCFAs) on small intestine morphology and epith elial cells proliferation in TPN-fed rats

    Abstract:Objectives:Total pareteral nutrition (TPN) has prov ed to be a valuable method of n utrtion support.However,when supplid as the s ole source of nutrition to normal anim als ,TPN produced atrophy of the small intestine. SCFAs were shown to stimul ate mucosal cell mitotic activity,this study determined whether the addition of SC FAs could prevent intestine mucosal atrophy produced by TPN. Methods: Thirty adult Wistar(RT1K) rats were clarified into three groups.With ten rats served as sham operation control .Tewty rats had catheters inserted into the superior vena cava,of which ten was sustained with classical TPN and o thers with TPN plus SCFAs(sodium acetate,prop ionate,and butyrate) for 14 days. Smll intestine villi hight,width,crypt depth,and epithelial cell proliferation index(PI) were study and compared. Results:Animals receiving only classical TPN h ad significant decreas of small intestine villi hight(162.8±18.3) μm,width(71.4±14.5) μm and cry pt depth(73.2±8.2) μm. Epithelial cell proliferation index(PI) was also very low(3.76±1.42) compared to control(5.51±2.14).In SCFAs-TPN trea ted rats, morphological parameters and prolif eration index were all much higher tha n classical TPN-fed rats.There were no apparent difference between SCFAs-TPN group and sham operation control group. Conclusions:The results suggest that SCFAs ma y be used to decrease the mucosal atrophy that commonly occurs after TPN support and other gastrointestinal inhibition state.

    Effects of enteralnutrient IMPACT on immune function of traum atic rat

    Abstract:Objectives:To evaluate the immunomodulato r y effect of enteral nutrient IMPACT,containing immunomodulator arginine-fish oi l and RNA,on the postraumatic immunosuppression of organism. Methods:The traumatic models were established by bilateral femoral fracture in 21 W istar rats.They were devided into 3 groups randomly according to the different n utrient:Ensure group,Nutrison group and Impact group.They were all fed intragas trically with each enteral nutrient which provides 837 kJ/kg per day for 5 days. The changes of body weight,serum albumin,peripheral blood T lymphocyte subsets a nd blood routine examination,quantitative microscopy of spleen were analysed. Results:Body weights and serum albumin levels of 3 groups decrea sed after trauma.In rats fed with IMPACT,the indexes of immune function were imp roved significantly. Conclusions:The posttraumatic immunosuppre ssion can be corrected by enteral nutrient containing immunomodulator.

    PEG——A new way of enteral nutrition feeding

    Abstract:Objectives:To evaluate the value of percut aneous endoscopy gastrostomy(PEG) in nutrition support of neurosurgical and neur ological patients. Methods:Thirty patients had undergone PEG. Results:Nutritional status was improved in the survival patients and the lung infections were controlled.Seven patients recovered with oral intake and 10 patient were discharge from hospital with the feeding tube for further rehabi litation,3 patients died from the deterioration of the original disease.Two case s had the inflammation at the PEG wound but easily treated.No other complications of PEG occured. Conclusions:PEG is safe and effective for ente ral nutrition in some neurosurgical and neurological patients.
