急性心肌梗塞延迟经皮冠状动脉腔内成形术对改善左心室功能的近期及远期疗效观察(摘要)

    中国循环杂志99zk21 目的:为进一步评价经皮冠状动脉腔内成形术(PTCA)血管再通血运重建对急性心肌梗塞(AMI)患者左心室功能的影响。

    方法:回顾性分析了我院1994~1997年间资料完整、符合WHO诊断标准的54例初次AMI患者住院期及随访8个月的二维超声心动图资料。根据是否施行PTCA治疗分为PTCA治疗组(32例)和对照组(22例)。两组在AMI发病后平均2~3周和8个月左右分别进行了心超检查,观察室壁运动异常情况并测量左心室射血分数(LVEF)。

    结果:PTCA治疗组和对照组的LVEF及室壁运动异常积分在住院急性期无明显差别,但在室壁瘤和左心室扩大发生上,对照组高于PTCA组。平均随访8.4±1.9个月后,再次心超检查发现两组心功能和室壁活动积分较急性期均有改善,PTCA治疗组心功能和室壁活动积分较对照组改善更为明显。

    结论:AMI病人发病后积极进行PTCA治疗,对左心室功能和室壁运动障碍的改善起到有益的作用。

    Effects of Delayed Percutaneous Transluminal Coronary Angioplasty on Left Ventricular Function in Patients with Acute Myocardial Infarction (Abstract)

    Objective: To assess the effects of delayed percutaneous transluminal coronary angioplasty (PTCA) on left ventricular motion and function evaluated by two-dimensional echocardiography (2-DE).

    Methods: Fifty-four patients with acute myocardial infarction were divided into delayed PTCA group and control group. Left ventricular ejection fraction (LVEF) was determined by Simpson biplane formula and left ventricular wall motion abnormality scores were also measured. 2-DE was performed after 2 to 3 weeks and 8 months of onset of acute myocardial in farction (AMI), respectively.

    Results: The results showed that ventricular wall motion abnormality scores and LVEF in delayed PTCA group in acute phases were the same as in control group. But the incidences of left ventricular dialatation and aneurysms in control group were higher than those of in delayed PTCA group (p<0.05). After 8.4±1.9 months follow-up, LVEF and ventricular wall motion in delayed PTCA group were better improved than those of in control group (p<0.05).

    Conclusion: Our data suggested that active percutaneous transluminal coronary angioplasty therapy after AMI can significantly improve left ventricular function and wall motion., http://www.100md.com
