蛛网膜下腔注射吗啡对剖宫产产妇寒战的影响

    临床麻醉学杂志000108 摘要 目的:研究蛛网膜下腔注射吗啡对剖宫产产妇寒战的影响。方法:剖宫产产妇100例,随机等分为两组,Ⅰ组实验组和Ⅱ组对照组。选用联合腰麻硬膜外麻醉,于L3~4间隙蛛网膜下腔注入腰麻液3ml(含布比卡因7.5mg),Ⅰ组腰麻液含盐酸吗啡0.5mg。记录阻滞平面、BP和HR,观察病人寒战发生情况。结果:寒战发生例数Ⅰ组26例,明显低于Ⅱ组37例(P<0.05);重度寒战Ⅰ组11例,Ⅱ组24例,Ⅰ组明显低于Ⅱ组(P<0.01);胎儿娩出前出现寒战者Ⅰ组4例,明显少于Ⅱ组15例(P<0.01)。结论:蛛网膜下腔注射吗啡可降低剖宫产产妇胎儿娩出前寒战发生率及总发生率。

    Effects of Intrathecal Morphine on Shivering in Parturients During Cesarean Section

    Abstract Objective:To determine the effects of intrathecal morphine on shivering during cesarean section following spinal anesthesia.Methods:Parturients undergoing cesarean section were randomly allocated in to group Ⅰ(n=50) and group Ⅱ(n=50).Intrathecal injection of 3ml solution consisting of bupivacaine 7.5mg with morphine 0.5mg in group Ⅰ or without morphine in group Ⅱ was performed at the L3~4 interspace.Analgisic level at 10min after intrathecal injection, and BP,RR and sedatioin score were recorded.Shivering was evaluated for a period of one hour.Results:There was no significant difference between the two groups in blocked level,BP,RR and sedatioin score.Twenty six parturients in group Ⅰ shivered compared with 37 in group Ⅱ(P<0.05).Eleven cases with severe shivering were seen in group Ⅰ and 24 in group Ⅱ(P<0.01).15 cases in group Ⅱ developed shivering before fetal birth compared with 4 in group Ⅰ(P<0.01).Conclusions:Intrathecal morphine reduces shivering in parturients during cesarean section,especially before fetal birth.

    管内麻醉后为体内热能从深部向外周再分布[1],因此剖宫产产妇寒战发生率较高。阿片类药物哌替啶、芬太尼等可降低寒战反应的阈值(即触发寒战反应的深部温度)[2~4]。本文研究蛛网膜下腔注射吗啡对剖宫产产妇寒战的影响。

    资料与方法

    拟行剖宫产手术产妇100例,ASAⅠ~Ⅱ级,随机等分为Ⅰ组(实验组)和Ⅱ组(对照组),每组各50例,均采用下腹部竖切口。两组病人年龄、体重、手术时间无明显差别。

    产妇左侧卧位,于L3~4间隙采用“针内针”技术蛛网膜下腔注入腰麻液3ml(室温),置硬膜外导管。腰麻液配方:Ⅰ组布比卡因7.5mg、盐酸吗啡(不含防腐剂)0.5mg加生理盐水至3ml,Ⅱ组布比卡因7.5mg加生理盐水至3ml。要求从注药开始到平卧位时间不超过4分钟,凡超过4分钟或腰麻失败者退出本研究,补充新病例。交替输注平衡盐液及胶体溶液,并根据血压调整补液速度。产妇入手术室后穿单层布衣、盖棉被,手术室温度控制在23±1℃,消毒后盖手术巾,手术开始后室温调为21±1℃。从注入腰麻液到胎儿娩出时间为15~30分钟不等,两组无明显差别。

    观察项目 用针刺法测阻滞平面,记录BP、HR变化,观察从注入腰麻液到手术开始后1小时内寒战发生情况。重度寒战指出现上半身肌肉颤动,寒战发生情况均用χ2分析。

    结果

    注入腰麻液10分钟后,阻滞平面达T11~T4不等,两组无明显差别。腰麻后BP略有降低而HR变化不明显,两组间亦无明显差别。

    Ⅰ组寒战总发生率52%,明显低于Ⅱ组的74%(P<0.05),重度寒战发生率Ⅰ组22%明显低于Ⅱ组的48%(P<0.01),胎儿娩出前寒战发生率Ⅰ组8%,明显低于Ⅱ组30%(P<0.01)(表1)。在本研究观察时间段内,无1例能够自行停止寒战。

    椎管内麻醉后体内热能从深部向外周再分布[1],体表散热、大量补液、冲洗及手术创面热量损失等原因使体温降低而引起寒战。产妇寒战发生率较高,可能由于妊娠末期子宫增大、血供丰富,与其他手术相比其能量再分布、冲洗引起的能量损失会更明显,从而使体温降低较为显著。

    阻滞平面多少及血管扩张的程度直接影响到体内热量再分布。本研究两组产妇阻滞平面并无明显差别,阿片类药物虽能增强局麻药的作用[5],但未发现蛛网膜下腔0.5mg吗啡有血管收缩作用,故本研究Ⅰ组的血管扩张程度和热量再分布不会比对照组低。

    在胎儿娩出前,手术对热量散失的影响很小。本研究从腰麻开始到胎儿娩出,对照组30%出现寒战,部份产妇在手术尚未开始即出现寒战,而蛛网膜下腔注射吗啡可明显降低这一段时间内的寒战发生率。由于平均体温、深部温度测量较复杂,我们未能测量,但从以上讨论可以看出两组病人体内热量再分布相同(或蛛网膜下腔组大于对照组)、散热程度相似,估计两组产妇体温降低程度近似,Ⅰ组寒战发生率所以较低,可能与吗啡通过脊髓或直接作用于体温调节中枢有关,而非通过减少机体能量损失发挥作用。
