眼轮匝肌颞区皮瓣的应用解剖

潍坊医学院000116摘 要：目的探讨眼轮匝肌蒂颞区皮瓣的解剖学基础。方法对15具成人尸体的30侧头面部进行肉眼解剖，观察眼轮匝肌的形态及血供情况，并对15例活体行颈总动脉数字减影血管造影(DSA)，观察颞浅动脉分支与眼轮匝肌营养血管的关系，然后将信息输入计算机图像分析仪行图像分析。结果①眼轮匝肌宽薄，分布广泛，血液由多个动脉分支供给，血运良好，位置表浅并以腱膜直接附着于皮肤，其中一部分可以被利用，使其可能成为肌皮瓣的蒂。②眼轮匝肌有多条血管在眼睑内形成动脉网，此动脉网与供应颞区皮肤的颞浅动脉分支——额支和颧眶动脉相吻合，且颧眶动脉位置较恒定，平均口径在1.0mm以上。③颞区皮肤质地、颜色、弹性较好。结论颧眶动脉是眼轮匝肌蒂颞区皮瓣的主要营养血管，以眼轮匝肌为蒂的颞区皮瓣可作为修复面部组织缺损的重要皮瓣。

Applied Anatomic Study on the Temporal Flappedicled with orbicularis Oculimuscle

Abstract：Objective To research the anatomy of the temporal flap pedicled with the orbicularis oculi muscle.Method The morphology and the blood supply of the orbicularis oculi muscle were studied on 15 cadavers by surgical dissection and vasclar cast.Using digital subtraction angiography(DSA),tpobserved the branches of the superficial temporal artery on 15 patients.Then the information was analysed by the computed image analysis syste.Result The orbicularis oculi muscle is wide and thin.It locates shallow.It is supplied by the braches of many arteries and the branchs have numerous connections with the branchs of superncial temporal artery.It is possible to get to a temporal flap pedicled with the orbicularis oculi muscle.Conclusion The zygomaticorbital artery is the most important vessel for the temporal flap pedicled with the orbicularis oculi muscle.
