大骨节病区与非病区儿童发中39种元素含量及Logistic分析

    中国地方病学杂志990612 [摘要] 目的 测定大骨节病(Kashin-Beck Disease,KBD)区与非病区儿童发中39种元素的含量,用Logistic统计分析,以探讨硒等化学元素与KBD的关系。方法 采用病例对照研究,用ICP-MS法检测KBD病区与非病区6~14岁5个组150名男童发中39种元素含量。结果 KBD区儿童发硒、硼、碘显著低于非病区,而镁、铁、锰、钴、钒、锂含量依KBD患儿、病区健康儿童、非病区健康儿童递减。病区铝、镍、钍显著高于非病区;Logistic回归筛选出钙、钠、锗、铜、铁、硼、锂、铬、硒、锑、铅、锡12种与KBD患病可能有关的元素。结论 除发硒含量低之外,KBD有多种营养元素和毒性元素的失衡。

    [Abstract] Objective Logistic analysis and content of 39 elements in hair of children between Kashin-Beck Diseased(KBD) areas and non-KBD areas were investigated in this paper in order to clear the relationship between chemical elements such as Se and KBD.Methods 150 children,aged 6~14 years old,males,in KBD areas and non-KBD areas were divided into 5 groups.Contents of 39 elements in hair were assayed by ICP-MS.Results Contents of Se,B,I in hair of children in KBD areas was significantly lower than that in non-KBD areas but Mg,Fe,Mn,Co,V and Li declined following children with KBD,health in KBD areas and haelth in non-KBD areas.Logistic regression were screened out 12 elements in hair related to KBD as follows:Ca,Na,Cu,Fe,B,Li,Cr,Se,Sb,Pb,Sn and Ge.Conclusions Several nutrient and toxic elements for human body besides Se were imbalance in hair of KBD children.

    大骨节病(KashinBeck Disease,KBD)是一种地方性慢性骨关节病。环境化学元素与本病的关系已进行了长期地广泛研究,较为一致的认识是分布在低硒环境中的KBD患儿发、血和尿硒含量均低,而硒以外的其他元素与KBD的关系尚未发现规律性[1~3]。本文报告了与美国加利福尼亚大学生物元素研究所合作测定陕西省彬县和永寿KBD区与非病区6~14岁男性儿童发中39种元素含量的变化,以及用Logistic分析初步筛选出12种可能与KBD患病有关元素的结果,为探讨环境元素与KBD的关系提供依据。

    1 材料与方法

    1.1 病区选择与研究分组 采用病例对照研究,在陕西省彬县、永寿县KBD区和非病区长安县秦渡镇1214名儿童中随机抽取150名6~14岁男童。病区KBD患病率为19.20%~41.94%。随机抽取彬县未采取任何防治措施的KBD区患儿和健康儿童各30例,口服亚硒酸钠片(每周1~2mg)的KBD患儿30例,永寿县服1/6万硒盐的同龄组健康儿童30例,并以长安县秦渡镇同龄组健康儿童30例作对照。每例儿童进行右手X线正位拍片检查,KBD儿童诊断依据“全国KBD诊断标准”诊断。

    1.2 元素测定 每例儿童取头枕部发样,盲法送美国加利福尼亚大学生物微量元素研究所,用电感耦合高频等离子发射光谱法(ICP-MS)测定39种元素含量,其中毒性元素15种:铝、锑、砷、铍、铋、镉、铅、汞、镍、铂、银、铊、钍、锡、铀;营养元素19种:钙、镁、钠、钾、铜、锌、铁、锰、铬、钴、钒、钼、硼、碘、锂、磷、硒、锶、硫;不属营养和毒性元素的其他元素5种:钡、锗、铷、钛、锆。

    1.3 统计方法 统计分析用SPSS 7.5软件完成。本资料数据用中位数(Md)、四分位距(Q)描述。经Logistic回归分析后,列出各元素的偏回归系数(B)、偏回归系数的标准误(SE)、复相关系数(R)及概率水平(Sig)。

    2 结果

    2.1 各组元素的含量分析 本次分析中,铍,汞,铂,铊4种毒性元素含量因低于最低检测限已被剔除。Kruskal-Wallis检验表明,除镉以外的34种元素均有显著性差异(表1)。(1)毒性元素:病区的铝、镍、钍含量显著高于非病区,而铀含量低于非病区(P<0.05);在病区内,KBD患儿发铝、铋含量高于病区健康儿童(P<0.05),而发锑含量却低于病区健康儿童,但病区各种毒性元素含量未超出正常限[4]。(2)营养元素:病区的发硼、碘、硒含量低于非病区。镁、锰、铁、钴、钒、锂的含量依次为:KBD患儿>病区健康儿童>非病区儿童。非病区发碘含量高于正常值。补硒后病区的发硒含量升高,但仍有23.3%的KBD病人发硒含量低于0.200μg/g。病区锰含量高于正常,其中KBD病患儿有88.35%高于正常值。非病区发钒含量正常,但KBD患儿有60%发钒含量高于正常。(3)非毒性及非营养元素:病区儿童发钛、锆含量高于非病区儿童。KBD患儿发锆含量高于正常值,与其它3组有显著差异。

    2.2 Logistic回归分析

    2.2.1 34种元素的Logistic回归分析 150名病区与非病区儿童发中34种元素的Logistic分析,初筛出与KBD有关的12种元素(表2)。将34种元素分为毒性元素、营养元素和其他元素三类后,分以下四种情况进行Logistic分析:A:全部观察儿童;B病区内健康儿童与KBD患儿;C:非病区健康儿童,病区患儿和健康儿童;D:病区补硒与未补硒的KBD患儿和健康儿童。Logistic分析筛出的毒性元素为:全部观察儿童筛选出铝、钍、锡元素;病区内健康儿童与KBD患儿筛出铝、锑元素;非病区、病区患儿和健康儿童筛出铝、铋元素;病区补硒与未补硒的KBD患儿和健康儿童筛出铝、钍、锡、铀元素。四种情况筛出一致的元素是铝。

    2.2.2 营养元素的Logistic回归分析 营养元素的Logistic回归分析见表3。全部观察儿童筛选出钙、钠、铜、铁、铬、硼、锂元素;病区补硒与未补硒的KBD患儿和健康儿童筛出钙、铬、硼元素。在其他元素类,全部观察儿童和病区补硒与未补硒的KBD患儿和健康儿童类似:钡、锗、铷、锆元素;病区内健康儿童与KBD患儿筛出钡、锗元素;非病区健康儿童、病区患儿和健康儿童筛出钡、锗、锆元素;四种情况筛出一致的元素是钡和锗。表2 150名男童发中34种元素的Logistic回归分析 变量

    3 讨论

    大骨节病发病过程中,维持儿童生长需要的体内元素平衡遭受病区环境因素作用后发生何种变化特点以及与KBD软骨损害的关系,是探讨本病病因研究的一个重要问题。本文报告了与美国加利福尼亚大学生物元素研究所合作调查大骨节病儿童发中39种元素含量变化的单和多因素分析结果。单因素分析重复出现既往全国报道的大骨节病区儿童发硒含量明显低于非病区。补硒后病区儿童发硒含量明显上升的结果[5,6],但仍有23.3%儿童未达到0.200μg/g。除低硒外,营养元素中病区儿童的发硼和碘含量也明显低于非病区,而发中镁、锰、铁、钴、钒、锂含量表现为KBD患儿>病区健康儿童>非病区儿童。这些结果表明病区儿童发中元素有贫硒高锰高钒高锶的状况。毒性元素中铝、镍、钍、铀元素也存在显著性差异。病区的铝、镍虽高于非病区,但并未超出正常范围。在其他类元素中,病区钛、锆显著高于非病区。KBD患儿发中多种元素的Logistic回归分析未见报道。150名儿童发中34种元素Logistic回归分析筛选出12种可能与KBD有关的元素,其中营养元素8种:钙、钠、铜、铁、硼、锂、铬、硒;毒性元素3种:锑、铅、锡;其他元素1种:锗。Logistic回归分析的结果进一步提示,KBD除发硒低外,还存在着多种元素的失衡。将34种元素分为毒性、营养和其他元素三类的Logistic回归分析表明,病区与非病区全部儿童、病区内补硒与未补硒儿童的两种情况筛选出的元素基本一致。在病区内,KBD患儿和健康儿童比较,硒不再进入方程,而筛选出铝、锑、铬、硼、钡、锗元素。

    根据以上结果,陕西彬县大骨节病区KBD患儿发中除低硒外,还存在多种营养元素和毒性元素的失衡。这些失衡的元素是否能在其它KBD病区重复出现类似的规律性,是否与KBD患儿血尿中元素含量变化相一致,以及在软骨损害中的生物学意义,还尚待深入调查。
