【摘要】 目的 探讨耻骨弓角度(SPA)与胎儿头围(HC)比值(SPA/HC)对孕足月产妇分娩方式的预测价值。方法 回顾性分析2023年7月至2023年12月于中山大学附属第三医院岭南医院产科规律产检并阴道试产的363例产妇临床资料。根据分娩方式分为正常阴道分娩组(291例)和手术分娩组(72例)。所有产妇均在孕足月后经超声测量SPA及HC。采用Mann-Whitney U检验比较2组SPA、HC及SPA/HC。SPA、HC及SPA/HC对产妇分娩方式的预测价值采用Logistic回归分析。采用受试者操作特征(ROC)曲线评估模型的预测能力。结果 363例阴道试产产妇年龄(30.33±4.40)岁，孕周(39.3±0.9)周。与正常分娩组相比，手术分娩组SPA更小，HC更大(均P lt; 0.001)。SPA对正常分娩呈正向预测作用，曲线下面积(AUC)0.739(95%CI 0.678～0.799)；HC呈负向预测，AUC 0.700

    (95%CI 0.637～0.763)；SPA/HC呈正向预测，且预测效能最优，AUC 0.817(95%CI 0.763～0.871)。结论 SPA/HC可有效预测孕足月产妇的分娩方式。当SPA/HC小于0.292时，建议加强产时监护并提前规划应急方案。
