【摘要】目的 探讨无创正压通气(NIPPV)在治疗急性左心衰中的应用价值。方法 对23例重度急性左心衰患者，在常规治疗基础上，加用NIPPV治疗，评价其临床疗效及并发症。结果 23例重度急性左心衰患者应用NIPPV治疗，19例病情好转，4例改有创通气或死亡，有效率达83.3%；对比过去5年中25例重度急性左心衰，常规治疗下有效15例(61.9%)，两组有效率差异有显著性，P<0.05。结论 NIPPV治疗重度急性左心衰不仅见效快，且抢救成功率高，值得推广应用。

Investigation on the applicative value of noninvasive positive pressure ventilation in acute left heart failure treatment

【Abstract】Objective the applicative value of noninvasive positive pressure ventilation (NIPPV) in patients with acute left heart failure treatment was investigated. Methods on the bases of routine treatment, NIPPV was administered in 23 cases with severe acute left heart failure, of which the therapeutic effect and complication were evaluated. Results of 23 cases with severe acute left heart failure who accepted NIPPV treatment, 19 cases got a turn for the better in their state of illness, and 4 cases had to use invasive ventilation even dead. The effective rate of NIPPV was 83.3%, which presented statistical significance (P<0.05, compared with 25 cases with severe acute left heart failure who accepted routine treatment during past 25 years, of which only 15 cases displayed effectiveness, the effective rate of which was only 61.9%). Conclusion NIPPV has a fast effectiveness and a high successful rate in treating and rescuing patients suffered from severe acute left heart failure. NIPPV is worth of application in an expanded field.

急性左心衰起病急骤，病情危重，常迅速合并呼吸衰竭，若临床上仅给予常规鼻导管低流量持续吸氧、强心、利尿和扩张血管等治疗，难以取得满意效果，常需行机械通气治疗，但会增加患者痛苦，产生呼吸机相关性肺炎等并发症，而且由于呼吸机疲劳对呼吸机产生依赖造成脱机困难，患者和家属均难以接受配合这种治疗。今年来无创正压通气(NIPPV)来治疗急性左心衰，越来越广泛地用于临床[1]，已被推荐为治疗急性左心衰的一线方法。我院于2006年1月至2009年7月采用NIPPV 治疗23例重度急性左心衰，取得较好的疗效，现报道如下。
1 资料与方法
1.1 病例资料在我院2006年1月至2009年7月救治的23例急性左心衰的患者为研究对象。其中男19例，女4例，平均年龄(67±11)岁；基础疾病为：肺心病合并冠心病9例，冠心病Ⅳ型7例，急性心肌梗死5例，风湿性心脏病例高血压病2例。呼吸类型：Ⅰ型呼衰14例，Ⅱ型呼衰9例。
1.2 病例选择应用(NIPPV)治疗患者具备以下条件：(1)急性左心衰竭(急性肺水肿及心源性休克)。(2)经常规利尿、扩血管、强心等治疗措施及鼻导管或面罩吸氧，脉搏血氧饱和度(SaO2)仍低于85%，甚至进行性下降，出现意识障碍。(3)血气分析符合呼吸衰竭标准 ......
