瘘管搔刮配合窦道通过术在治疗有瘘型慢性根尖周脓肿的临床观察

【摘要】目的 观察瘘管搔刮并配合窦道通过术的运用在治疗有瘘型慢性根尖周脓肿的临床效果。方法 选择160例患有瘘型慢性根尖周脓肿的病例，共160颗患牙，随机设为实验组(80例)对照组(80例)。实验组患牙常规根管预备清理同时采用瘘管搔刮并配合窦道通过术治疗，对照组只采用常规根管预备清理，不予瘘管处理。术后两组均在根管封存甲醛甲酚消毒棉捻。7天后对两组患牙进行临床观察和比较。结果 实验组72.5%(58/80)封药一次后即可进行根管充填术，对照组46.3%(37/80)封药一次后可行根管充填，两组患牙在封药一次后的根管充填率存在极显著差异(p<0.01)。实验组在封药3次后共完成根管充填总数为78例，充填率为97.5%；对照组在封药3次后共完成根管充填总数为71例，充填率为88.8%。两组患牙总根管充填率存在显著差异(p<0.05)。结论 瘘管搔刮并配合窦道通过术促进了瘘管的闭合，减少根管治疗封药次数，有利于有瘘型慢性根尖周脓肿的尽快愈合，提高疗效，缩短病程。

Clinical effect of fistula curettage and sinus flushed on chronic periapical abscess with fistulas.

【Abstract】Objective To observe the clinical effect of fistula curettage and sinus flushed on chronic periapical abscess with fistulas. Methods Choosing 160 teeth of chronic periapical abscess with fistulas, they were divided at random into a test group(n=80) and a control group(n=80).The test group was implemented conventional root canal preparation and clean-up with fistula curettage and sinus flushed. The control group was only implemented conventional root canal preparation and clean-up without fistula curettage and sinus flushed. The both groups were disinfected using formocresol cotton in root canals, then clinical observation and contrast were continued between these 2 groups after 7 days. Results 72.5% in the test group could be started root canal filling after the first therapy, but 46.3% in the control group could be started root canal filling after the first therapy. Extremely significant difference (p<0.01)existed between the rates of root canal filling after the first therapy. 97.5% in the test group could be started root canal filling after 3 times therapy. 88.8% in the control group could be started root canal filling after 3 times therapy,statistical difference﹙p<0.05﹚existed between these 2 groups. Conclusion Fistula curettage and sinus flushed can decrease times of root canal disinfection and a better result can be obtained on chronic periapical abscess with fistulas.
