脾切除加贲门周围血管缝扎术治疗门静脉高压消化道出血的临床效果分析(1)

【摘要】目的 探讨采用脾切除加贲门周围血管缝扎术治疗门静脉高压消化道出血的疗效，以供临床参考。方法 对我院2009年8月~2011年1月收治的39例门静脉高压消化道出血的患者进行脾切除加贲门周围血管缝扎术治疗。比较患者治疗前后的门静脉压力、血清Tuftsin、血清免疫球蛋白IgM。观察患者治疗后的并发症发生情况，并将结果进行统计学分析。结果 39例患者经过治疗后，其门静脉压力明显降低，与治疗前相比，p<0.05，差异有统计学意义。患者血清Tuftsin无明显变化，血清免疫球蛋白IgM无明显变化，与治疗前相比，p>0.05，差异无统计学意义。39例患者均得到良好的治疗，患者无死亡。共发生并发症5例，占12.82%。结论 采用脾切除加贲门周围血管缝扎术治疗门静脉高压消化道出血具有较好的疗效，患者并发症少，值得在临床推广使用。

【Abstract】Objective To investigate the use of splenectomy plus ligation cardiac peripheral vascular treatment of portal hypertension gastrointestinal bleeding, for clinical reference. Methods From August 2009 - January 2011 were treated 39 cases of gastrointestinal bleeding in portal hypertension in patients with splenectomy plus ligation treatment of peripheral vascular cardia. Compared before and after treatment in patients with portal vein pressure, serum Tuftsin, serum immunoglobulin IgM. Observed after the treatment the incidence of complications, and the results were statistically analyzed. Results of 39 patients after treatment, the portal vein pressure was significantly reduced compared with before treatment, p <0.05, significant difference. Serum Tuftsin no significant changes in serum immunoglobulin IgM, no significant changes compared with before treatment, p> 0.05, the difference was not statistically significant. 39 patients have received good treatment, patients with no death. Complications occurred in 5 cases, accounting for 12.82%. Conclusion splenectomy plus ligation cardiac peripheral vascular treatment of portal hypertension gastrointestinal bleeding with better efficacy, fewer complications in patients, it is in clinical use.
