五种宫内节育器在人群中使用状况及避孕效果分析(1)

【摘要】目的 了解人群中常用宫内节育器(IUD)使用状况及避孕效果，分析失败原因以指导避孕节育知情选择和随访服务。方法 采用分层整群抽样方法收集23074名妇女近期避孕动态，用寿命表法分析IUD总终止率及原因别终止率。结果 金属单环、TCu220C、活性金属环型165、宫铜200、活性r型为使用最高的前五位。有7.26%正在使用IUD妇女无法获知所用IUD类型；主管部门要求停止生产和使用金属单环后，仍有放置金属单环的情况发生。金属单环、TCu220C、活性金属环型165、宫铜200、活性r型等IUD的10年终止率、带器妊娠率、脱落率以金属单环最高，分别为34.23%、8.75%、10.35%,显著高于其它4种IUD(P<0.01)；TCu220C的10年累计终止率、带器妊娠率、脱落率位列其次，分别为17.43%、8.01%、9.04%。结论 金属单环的带器妊娠率、脱落率高，TCu220C的总终止率、带器妊娠率、脱落率均较高。应注重妇女使用IUD类型等相关信息记载，加强对淘汰与暂停使用产品的处置与监督，以提高育龄妇女的生殖健康水平。

【Abstract】Objective To understand the use situation and the efficiency and safety of intrauterine devices (IUDs), in order to provide the scientific basis for improving family planning service. Methods Survey the information of contraceptive dynamic based on stratified cluster sampling. Using life-test to analysis discontinuation rate and Log-Rank X2 for rates significant test。Results: The use of top five types IUDs are Single-ring IUD, TCu220C, Uterine Cavity-shaped 200, Medicated r-IUD and OCu165. 7.26% of women did not know the IUDs type which was being current used; Single-ring IUD were still inserted after it was asked to stop using by government; Compared with TCu220C, Uterine Cavity-shaped 200,Medicated r-IUD and OCu165,10-years discontinuation rate(34.23%), pregnancy rate with IUD(8.75%) and expulsion rate (10.35%) of Single-ring IUD is significant highest(P<0.01); 10-years discontinuation rate(17.43%), pregnancy rate with IUD(8.01%)and expulsion rate (9.04%) of TCu220C is significant higher than the other three types IUDs.
