乳腺癌中趋化因子CCR7CCL21表达与淋巴转移关系的研究(2)

肾小球足突细胞膜表面黏蛋白Podoplanin是特异性的淋巴管内皮标记物之一,相对分子质量为38103,它仅表达在淋巴管内皮,而在血管内皮中不表达，因此成为目前公认的淋巴管内皮细胞特异性标志物之一[13、14]。最近研究表明结直肠癌组织中存在新生淋巴管,且主要分布于肿瘤边缘区,癌周围淋巴管密度增加与癌细胞转移相关，我们的研究也证实在乳腺癌内部和肿瘤周边的正常区域存在Podoplanin染色阳性的淋巴管内皮细胞，而且多数为新生淋巴管，内皮细胞较大，基底膜不完整，淋巴管周围存在多量炎症细胞浸润，间质水肿，间质中炎症细胞表达趋化因子，而且我们研究显示淋巴管密度与乳腺癌的淋巴转移密切相关，因此Podoplanin阳性表达的新生淋巴管参与乳腺癌的淋巴转移。
尽管很长时间以来，人们假定淋巴道转移是一个被动过程，肿瘤细胞黏附状态改变，肿瘤细胞脱落后到达淋巴结是经过已经存在的淋巴管道，与新生淋巴管无关，但最近多数研究显示淋巴管生成对于促肿瘤转移是很明显的。我们的研究尽管证实CCR7 CCL21的表达通过促进乳腺癌中淋巴管生成间接导致肿瘤淋巴转移，将来对趋化因子与淋巴转移之间的关系及其具体机制的深入研究可以为寻找阻断肿瘤的淋巴转移提供新的靶点。
