4层螺旋CT的VR\MPR对门静脉血管重建技术的探讨

     [摘要] 目的 探讨4层螺旋CT的VR、MPR对门静脉重建的技术。方法 收集肝脏平扫加增强扫描80例,着重对肝脏平扫CT值的研究来确定门静脉对比剂延迟时间,应用三维容积再现(VR)与多层面重组(MPR)两种重建技术门静脉重建技术的研究。结果 VR和MPR重建图能清晰显示病变位置、范围,血管和周围邻近关系及侵犯情况,使病变情况观察更直观。平扫肝脏CT值越低,门静脉扫描延迟时间越长,正常CT值组45s时门静脉成像效果一级图像高达80%,偏低CT值组50s时门静脉成像效果一级图像达60%。结论 门静脉延迟时间的掌握,对比剂总量和流速的测量,都是CTP高质量图像的重要环节;大多数患者延迟时间45-50s可获得较理想成像效果,更好为的VR、MPR等后处理技术提供保证,为临床提供更多直观、准确的有关门静脉的信息。

    [Abstract] Objective To evaluate the reconstruction techniques of VR, MPR for the portal vein. with 4 silces spiral CT.Method Eighty patients were performed a 4 slices spiral CT together with enhancement. Portal vein and their branches were constructed by volume rendering(VR) and multiple planar reformation(MPR) techniques.The delay time of the portal vein were evaluated with the CT value of the hepatic parenchyma on pre-contrast image. Result Portal vein branch could be revealed by VR and MIP technique clearly,which could directly demonstrate the location and the extent of the lesion,and show the relationship and invasion with surrounding structure. The lower CT value of the hepatic parenchyma, the longer of delay time of the portal vein.If the CT value (of hepatic parenchyma of pre-contract) was normal, the delay time of portal vein was 40s, and the display rate of one-level image were up to 80%. The delay time (50s) was longer and the display rate of one-level image (60%) was lower when the CT value decreased. ConclusionTo get high quality images, the delay time of portal vein controlling, contrast agent volume and flow rate measurements are all the important parts of CT portography(CTP)and the recommended delay time of portal vein was 40s to 45s. Also the reconstruction techniques ; the VR, MPR ect, could directly and exactly show the information about the portal vein for clinical.
